Thanks for sharing. The photo is simply of a very large transscrotal piercing. They're prone to infections for obvious reasons, but a lot o' guys think they're kewl. Only the most skilled piercers will usually attempt to undertake them. I personally think he's still got his balls, but I can't be totally sure.

I was doing body piercing when the first successful trans-scrotal piercings were being done. Most of them are scapeled and then the two surfaces of the scrotum must be stitched together. If one has a loose enough sac, he could have the huge flesh tunnel installed as initial jewelry, but that would probably create healing issues.
