It sounds like he has a sexual fixation of some sort. The self-treatment of the fixation is a result of using sex to self-treat a functional case of depression. Sex becomes the object of living. It would be a form of sexual addiction. He needs a therapist in a bad way. If you only live for sex you do not have a very fulfilling life.

The above described me to a "T" (pun intended) before castration.

I was not "gay" but I was bisexual before castration. Jesus, it sounds like this fellow lives for sex and is deeply depressed on top of that. To him, castration is suicide. Not sure how he can be helped, since he will die from the cancer if he is not treated. Tough case no matter how you handle it.... Prayers.
