Never liked Freeform. Ruins the game for me when 'anything goes' and anything you come up with can be easily un-done... There is no strategy/challenge, no danger, no way to lose, and effectively no way to win.
Got to have rules and a play-system as guidelines, but with some flexibility (a rules lawyer, or too many rigid rules, is also not much fun).
I've tried both online and in-person, have to say I like in-person a lot better. Or better yet, in-person augmented by technology (ie: Laptops or PDAs/Tablets which keep track of things like GP, HP and Inventory).
If I were DM, as far as healing goes, it would depend on the nature of the injury.
If it was from being pounded/crushed by a mace I would treat it like any other injury and allow standard heal spells such as Cure Serious Wounds to heal it.
If it was burned off by acid, or actually cut off, I would require some means of limb regeneration such Regeneration (Lv 7 Cleric Spell from DnD 3.5 and/or d20 SRD).
Although, making a quest to restore the missing bits is a really neat idea!!
We had a eunuch in a game once years ago (DnD 1st edition). Trick magic item called a "Belt of Feminine Masculinity" which appeared to be a "Belt of Giant Stregth" but as soon as the character put it on, it changed thier sex, with a 10% chance of removing thier sex. Well the guy rolled an 09... So his character became a eunuch! (for more info see: http://en.wikipedia.org/wiki/Girdle_of_ ... asculinity )
I've played 1st, 2nd, 3.5, and Pathfinder ("3.75"). But have never played 4th edition.

I've never played an in-person game before. I agree I'm not all that fond of freeform- It's great if there's only 2 or maybe 3 players and everyone agrees one what's allowed and you're just trying to tell a story, but that's about all. I've only ever actually played 3.5 and D20 modern, but I know both WoD systems, GURPS 4e, Pathfinder, and D&D 4e.
