Letter to My Doctor for an Orchiectomy

This is a copy of a letter given to my Doctor in regards to getting an Orchiectomy for a better Quality of Life. My Doctor advised the Urologist to proceed after this letter was given to him in person and we discussed it. It was performed 3/11/05. I hope this may help someone write a similar letter to get what they need to live a Great life.

To Whom It May Concern:

Currently I have been on Andrucor 100 mg/ day since 12/23/2003 for one month and continuing 50 mg/day. So far the results are as follows.

1. In addition to an infection I have had a urine flow problem and frequency issue. With prostate issues starting to surface my fear of cancer is strong.

2. Prostate cancer is a problem at my age and Andrucor includes Prostate cancer reduction.

3. My test Testosterone level is currently 22.4 NG/DL PSA < 0.1 (PSA in 2002 0.6)

4. I feel positive with a new calmness and a major reduction in my libido.

5. Libido reduction is another reason for reducing my testosterone level to castration levels.

6. In order to eliminate the side affects of medical treatment, Andrucor or other chemical, it is my desire to have an Orchiectomy after a year or so of chemical castration. My Urologist will do the procedure after your analysis of my situation.

7. It is my desire to be more asexual in nature. I have always been physically small and not a good sex partner; however during my marriage we found a way to make things work out. At my age I don’t want to go through making things work out and have no desire to do so.

8. Sex is not the most important part of my quality of life. I have many friends male and female and forming a marital partnership is not of interest to me.

I understand that there are some side affects of an Orchiectomy such as bone loss and possible negative issues. I have been taking 830mg calcium + D per day and 17b estrogen .75 mg transdermal.

So far my attitude is positive and I have a new calmness when working with people because I am not thinking of sex due to a libido out of control most of my life, before and after raising a wonderful family. My wife passed away 3 years ago.

During the last 6mos, I have become a Certified Computer Professional and believe to become more successful and maintain a positive attitude, continuing education and training is important to me.

Please advise me if we can work together to achieve my goals outlined above.
