New Doctor

Well, just as many suggested I should do, I finally got around to firing my doctor. I now have a new doctor who I go to see on Friday.
What should I say in order to have the best chances of convincing her to prescribe hormones?

well, for one, tell her that you're self medicating and want to do this "the right way"
for 2, be pleasant.
Show her that your transition is a level headed decision, and that you're not a loony. If you come off educated on the subject, and confident you're making the right choice, then she will be more inclined to treat you in the way you're seeking. Request a blood test, tell her you're concerned about your health and want to make sure your body is not being harmed. Also, make sure she knows about your gender therapist that you selected. A mental health supervisor is another good way to show you're in good hands. If she abides by the Harry Benjamin SOC, then she may or may not perscribe horomones, but she will at least be able to monitor your bloodwork, to make sure the self medicating is not harming you.
All of this will prove that you're thinking of this long term. All of these things will make her more confident that you're moving in a positive forward direction. Good luck!

What I'm thinking could likely happen is what's happened with other doctors - she'll say it's beyond her area of expertise and she has to refer me to an endo.
The problem with that is if she lists TS as the reason for the referral, the insurance will not approve it.
What can I say to her if she wants to refer me?

I would just explain your situation and why she must not send you to the endo. Be up front with her about the insurance and all. Doctors dont know squat about insurance, so your gonna have to tell her.

Just ask for a horomonal blood test, dont bring being TS up... that just adds to problems.

Since I was able to get hormones from another doctor, I don't have to worry about it anymore
Now I just have to figure out how to convince her to give me SSRIs without wanting to send me to a psychiatrist.

Plix you may have to see a psychiatrist i know i did when i asked my doctor for premarin but if you are honest and tell them how you feel and why then remember they are their to help you.So my advice to you is to be honest and dont be afraid just be yourself.

Once again sag has it right. Trying to mislead your doctor is a really effective way of making trouble for yourself because it prevents the doctor from doing his best for you. Anyone who is afraid of telling his doctor the truth needs a different doctor---or a different attitude.
