Looks like the Belgium-based Operating Room Roleplay is for real. Not only are people here posting about going there, but check out their web site.

At least 2 people regularly posting to the EA or to related Yahoogroups have obtained elective bilateral orchiectomies there. Cost for an orchiectomy is 2000 euros, about the same as Dr Kimmel in Philadelphia.
