What routes do you guys take to get a penectomy? Do you guys get a cutter or do you do this yourselves or is this something that is impossible to achieve in the USA?

Well, from what I've read here--and that's pretty much all I know about it--it's quite an ordeal to get it done here in the USA, but one news item posted here recently reports that a teenager shot his penis accidentally. I suppose that if a guy is desperate to get a penectomy in the USA without the hassles others have reported, the quickest route might be to "accidentally" (on purpose) shoot his penis off, then destroy it before heading to the hospital, claiming when asked for the missing part that he didn't think anything could be done with it so he ran it down the garbage disposal. From what I've read here on EA, that might be the only route to get it done in the USA without spending a lot of time with a psychiatrist.

Doctors/surgeons in the U.S. take the Hippocratic Oath which prevents them from doing bodily harm, that's why you can't walk in, ask for a penectomy, and have them do it. That's why we tell people to go to Thailand or India, there, if you have the money you can get it done. It's the safest way to prevent death. However, it won't stop people in the U.S. from trying other back alley methods.

The main approach is to damage it yourself to a degree that proper surgical removal is necessary. I believe YodaNell did that (you can read his account and see pictures). He banded until it was dead.
I don't recommend this of course, but it seems to be the best balance -- i.e. possible to obtain, but proper surgical result.
Any self-cutting is extremely dangerous. The blood supply is just too significant and the nature of the structure (i.e. it is not just normal muscle, but rather spongy erectile tissue) just makes a full do-it-yourself too dangerous (truly life threatening).

I was surprised after watching Extreme Slave11 cut off his glans (malebodymods.com), that it didn't bleed more. I had supposed that after watching him cut it off, the blood would have spewed out like turning on a faucet or opening a valve, but that didn't happen. Extreme Slave11 did say that he used a special clotting bandage that he bought off the internet, which helped a lot, and it prevented him from going to the ER. I'm not sure what would have happened had he cut his whole penis off at the base. If you haven't watched his video, it's vivid & gruesome, but it shows you what some men have to do. He simply uses a knife and cuts the head of his penis off. Fortunately, he had prepared and was ready for the aftermath.

Cutting the penis off at or near the base must be an extreme risk unless professionally done. On the other hand removing the glans seems a lesser risk since you don't see those great big veins bulging out. Not knowledgeable about either process, but personally would like to have my glans removed. Any suggestions on where to get it done safely?

I believe the glans is somewhat compatrmentalized and has less significant blood flow. Basically if you think about it, the blood that goes to the glans is probably only about 1/6th the amount flowing through the base of the shaft. Also, the glans is sort of regular flesh, while the shaft is a spongy erectile structure -- the latter may have more difficulty stopping bleeding once it starts, whereas the glans probably just does normal coagulation and vein/artery collapse. Lastly the glans is the end of the penis, so I believe the blood gets distributed into it via smaller arteries.
One thing to keep in mind too is that when flaccid you won't see much blood flow at all, even if you sliced off the whole thing -- that's the point of being flaccid. But as soon as your body gets sexually excited, it will pump full force. So when his glans was cut off it might not of bleed immediately, but who knows how much it spurted and weeped later on.

For the record, when he cut his glans off he did cut into the cavernosa a bit. You can see it on his healed photos. He cut through them where they narrow and go into the glans.
Even with that, he didn't bleed much. Blood has a habit of looking like it's a lot more coming out than there actually is.

Loosethem is correct. When I did my glans separation I cut the tops off my cavernosa and although I did small cuts over a week it didn't bleed much. Also the glans is very spongy which is why it swells during an erection. Splitting my glans vertically bled just as much as cutting through the tops of my shaft.
