Surgery always sounds so simple -- MTF transsexuals will hear how they can have a penis constructed from skin and muscle from other parts of the body. The reality of surgery is always a lot more brutal when you really look at it. I think this picture is of them constructing a fake penis from the forearm

Oops, yeah. But either way the point is that surgery always seems like "oh you'll just go to sleep and wake up with new genitals, face, without a uterus, etc." but even the simplest surgeries are extremely intrusive.

You'd have to ask them, and they may all have different reasons. I expect that it is expensive, very intrusive, and the end result is fairly unsatisfactory. For example, the constructed penis cannot get erect unless they further put in an implant that has to be literally pumped up before sex.

I think many FTM get a somewhat decent effect from testosterone alone. If you look at the clitoral enlargement of female body builders (look of nude pics of Denise Masino) they can actually achieve some penetrative sex with their enlarged clit and it is fully functional in terms of erection and orgasmic sensation.
