Sex and surgery in general are not compatible activities, because good surgery requires an aseptic environment to reduce the risk of infection and sex for the most part including S&M tends to be anything but aseptic.

I'm thinking you mean "safe" and not "save" in the title?

I am really not an expert on this, but I would not suggest doing something in play that a doctor should do.

You should let this idea remain a fantasy that you enjoy. Doing something like this in an unsterile environment is dangerous

The fantasy is real but in reality there is only one safe way to be castrated. I would suggest that take your time and study and learn everything you can about castration, its ups and downs. There are many people here that can help you with that.

After Flo's recent hospital visit, I wouldn't even call the professional route safe.
