Insurance and Elective Eunuchs

This thread is sort of a break off from another where the issue of telling a doctor that you are an elective eunuch is being discussed.
I may have the opportunity to get some insurance as soon as Ken gets a full-time position with the company he works for (and it looks like he will before too long). At that time I will have the opportunity to see a new doctor and perhaps have my T covered under the insurance.
But there may be a problem, and I am wondering if any other elective eunuchs have come across this problem with their insurance.
We know that most insurance plans decline any coverage for elective treatments, which an elective castration would probably qualify under. And most also decline coverage for any consequences that arise from the elective treatment. Presumably the need for T replacement would be a consequence of said treatment.
So my question is will T even be covered for me at all if the insurance company finds out that my surgery was elective?
If it will not be covered, what can I tell the doctor in order to have a better chance of coverage? Has anyone else come across this issue?
