I'm pretty sure if any thing is going directly into the bladder and bypassing the bladder's sphincter, then diapers would be required, unless some other means such as an external valve were used on the catheter. I tried to find a good diagram on the internet to show what I am talking about. Perhaps some one else can post a link.
In any case an procedure such as this would not be for the faint of hart.

A lot of older people have their urethras relocated to Florida. But it's just too humid there for most people.

If the original urethra is blocked, the ejaculate goes retrograde into the urinary bladder. The ejaculate product is then excreted with the urine.

A suprapubic catheter is usually an extremely small gauge and only urine will pass through it. In theory, a foley catheter can be inserted with a suprapubic technique but infection is usually, sooner or later, the result. Unlike a "G" tube or a gastric tube, the urinary tract is normally a sterile environment while the Gastro-Intestinal system is not. So, "G" tubes can be easily healed without infections. Loop illeostomies are usually used when cancer (or serious trauma) necessitates the removal of the bladder and or ureters. However, as posted above, if there is no muscle controlled spincter, the urine drips out as it is secreted and must be caught in a bag, an osteomy bag.
All sorts of surgical techniques have been done in the past and if you are interested, seek a medical school library and look at specialty urology textbooks. However, misery is usually the end result.
These techniques are generally used to buy time for terminal cancer patients and are used on severe injuries to keep people alive. Because of their gruesomeness, they are not seen often.
If you are getting a fixation on this you need to really consider talking to a professional. If is it just a fantasy, well, just don't get carried away, O.K.?

I have personal experience with urethral relocation. Mine was relocated behind the scrotum. It wasn't for health reasons, it was for my own personal reasons. I had to undergo 12 operations for it and may end up with a 13th to widen the hole, but otherwise, I'm very happy with it.
