I don't believe this is photoshop. It is actually possible to do this.

and then work the two photos together.

Well... I think either possibility makes this picture quite remarkable. If it's real the model is quite remarkable but if it is photoshop it is very well done. I'm leaning toward real. As you say the shadow looks perfect. Also there are no soft blurries that give it away.
