When a penis becomes erect, blood is allowed in but not allowed out. I believe that there is an operation that is used to cure priapism (a continuous erection) that permanently opens the valves that keep blood in the penis.

I was on a drug called prednisone for 12 years for an autoimmune disorder. Since the first year I was on it I haven't had an erection. That was 1984. But I'm not sure how you'd get any without being at death's door with a truly dreadful imflammation of some kind (mine was crohn's disease) The other side effects are quite awful, but it stopped the internal bleeding that threatened to kill me.
