Perhaps playing a game where the doctor pretends to consult the couple regarding the of extracting male energies from the husband, perhaps by removing just one testicle to begin with (while really this is all pre-planned with the wife). The wife pretends not to want it at first "wanting to keep her husband a real man", but the doctor persuades her, by making them standing naked next to each other and fondling both pairs of testicle, telling the wife something like "see, your testicles are almost as large as your husbands already, feel them... I don't see any reason why your husband could not donate one testicle for a faster growth of your's"
