Castration, TG vs Fetish

It seems to me that transgendered people who undergo orchiectomy seldom have any of the severe problems post-op that those who undergo surgery for what I would call "fetish" reasons do. Do you think this is more likely psychological, or is it chemical (due to the fact that most TGs take hormones anyway.)
I think it's both, personally. Certainly the hormones help, but also I think a lot of it must be mental. Most transgendered people undergo orchiectomy as a means to an end (ie no more testosterone, reduced body hair, etc) but for fetishists, it's the SURGERY that is important. Post op, they have nothing left to fantasize about, and I'll bet that this is the source of a lot of the depression and unhappiness common to those who pursue castration as a sex thrill.
Personally, I cannot imagine ever doing something this irreversible as some kind of kinky game. I worry about some of the people I see in the personals, who say things like "I want to get my balls cut off by a master who will make me his permanent sex slave." What happens when,post op, most of your sex drive is gone? Will that still sound like a fun situation? Or what happens when so and so gets bored with you, or real life intrudes. I realize that most of it is fantasy and (I hope) most people really wouldn't do something like that, but it's something to think about.
It would be interesting if somebody did a psychological study on those who want themselves to be surgically emasculated, along with all the stuff that seems to go along with it (Make me your bitch, treat me like a dog, abuse me) to see what went off during their sexual developement. There are many, many works on transsexuals, but few it would seem on other variancies of this type. Anybody know of any I may have missed?
