What are the chances of a male in his early twenties getting testicular torsion in both testicles at the same time?
What about at different times over the period of a few months?

My own experience with life in general is that other people are actually much less interested in one than one thinks. If you are seen in the shower, for instance, sans balls, assuming anyone even notices, they'll think, "must have had some kind of medical emergency" and turn their thoughts back to their own affairs, which are much more interesting to them than anything regarding someone else. They won't think "voluntarily castrated?" because elective castration, so important as either a fantasy or reality to so many members of this site (including myself), just isn't part of their conceptual universe. And even if they (e.g. parents or siblings) are interested, they will probably assume the matter is too delicate to raise. All the best.

My personal experience is that you're absolutely right. I've been nude in social situations much of my life and decided to follow through with my longstanding desire to be ballless 4 years ago when I was quite certain that I'd not let their absence deter me from being nude. I go to nude beaches, house parties and even nude or clothes-optional dance clubs in addition to showering at fitness and athletic events. That I've still got my sac may keep many from noticing. For those that do notice, hardly anyone ever says anything. Those that do - mostly at clubs - say things like 'neat', 'kewl.' I know of other ballless guys that have had similar experiences.

Small, tiny, little...chances, that is...
1) Tell them you're a yoga fanatic and got twisted up whiole trying to achieve a twisted and very advanced position.
2) Use the old excuse, you fell in the shower and landed on a can or bottle.
3) you jumped into a swimming pool and jammed a submerged object into your crotch

Tell 'em it's a war wound from somewhere in southwest Asia and you really can't talk about it.
For me, I've yet to be in a naked place with others, but so far I've been up front with all who need to know. Without any embarrassment, I just tell them I no longer wanted anything to do with sex. I've not had any negative feedback yet. On the contrary, everyone, from family and friends to medical persons, has been supportive. With my one friend, I told him now I know why geldings are often seen looking glazed eyed up at the sky, not bothered by anything.
Strangers would have to be looking pretty hard and for a reason to see the results of someone's castration. For the more daring of them, "Wow! What happened?" A shrug and tip of the head without a word should handle it.
