Seeking Advice to Orchiectomy Dilemma

To ensure anonymity, Im going to keep the backstory brief. A healthcare system that provides my care approved me to get an orchiectomy. But they refuse to do it in-house and have attempted to get doctors in the local area or beyond to do it. Its a conservative part of the country and all the doctors so far have refused to do the surgery, probably because its being requested for gender dysphoria.
Im really suffering with testosterone levels that have been back to something near normal male levels for months after years on HRT. Im popping hair on my breasts, etc. The suffering isnt garnering much sympathy.
My idea is to force an emergency orchiectomy surgery due to my testicles being damaged beyond repair by some sort of action. Thats what I need help with. Please hit me with all your ideas. The best ideas will be ones that do maximum damage to my testicles and pose the least to the rest of me. I dont want to bleed out while Im unconscious.
I plan on being very near an ER when I execute out this plan.

Welcome, and I hope we can help you with your dilemma...
However I would STRONGLY advise against the course of action you are proposing, as there really isn't any sort of action you can do that would damage your testicles enough to make the docs willing to remove them, without also having serious risks... It also seems likely if the local docs are as conservative as you say, that they would be more likely to attempt to fix them than to chop them off... It would also probably land you in the psych ward for a while...
Trying to get friends to help you is also problematic as if it can be shown that they did, or even knew ahead of time what you were going to do w/o trying to stop it, is very likely to get them into legal trouble.
Difficult, but it is worth noting that if you have gotten approval for an orchie, it is almost certainly a GROSS violation of medical ethics for a competent doc (as in having the appropriate training and knowledge - i.e. any urologist) to refuse to provide the surgery - your diagnosis is irrelevant. IF you are willing to take the action, filing complaints with your local and state medical boards (and or talking to your friendly lawyer) could cause serious trouble for doctors refusing to do the surgery... Downside is that it will "OUT" you to at least some degree, which may be a problem for you given your desire for anonymity... The other aspect is whether you really want an unwilling (and probably pissed off) doc carving on you?
Possible other options.... Even in a conservative state like yours, I'm sure there are Transgender centers that can perform the surgery, or you could also look into "medical tourism" as there is a clinic in Mexico that many EA members have used and been very happy with.... I'd definitely say that travel is a better choice than trying to do damage to yourself...

I am not dysphoric, and I still take T, but I had a similar dilemma. My testicles stopped producing T at normal levels years ago. I had been on TRT for years, so my testes atrophied, were prone to cysts, and I had a bout of epididymitis. I felt that since they weren’t producing, and I was getting my “balls in a bottle” for years, I wanted them out. I went to one urologist who just blew me off (figuratively, that is), so I ditched him. I decided to inject my testes with Everclear pure grain alcohol (although I hear vodka works just fine), then went to another urologist complaining of aching and discomfort when I walked (not necessarily true but with a purpose). He ordered an ultrasound, and the results showed hypoechoic lesions, along with atrophy and cysts with a suspicion of neoplasm (cancer). The doctor gave me two options, ultrasounds every 3 months to monitor them, or castration. I told him I would think about it, waited a few days, they told him I wanted a bi-lateral radical orchiectomy, with the biggest implants he could find! I knew it wasn’t cancer, but I said that I didn’t want to wait 3 months and then find out they were cancerous. He agreed to it, the insurance approved it, and I was castrated in 2014. The implants aren’t much bigger than adult-sized, so that was a disappointment; I wanted horse testicles! LOL! You wouldn’t harm yourself using alcohol injections, and it will mimic cancer, so you are guaranteed castration and your insurance will pay for it. It only took a couple of injections to do the job. My first question is, Are you in the USA? And if so where in USA? there should be no problem if you are over here. More and more doctors will do this surgery and some insurances will pay for it. Some doctors will require two letters from Psychiatrist or Psychologists.

Alcohol is probably one of the safer approaches (BTW Everclear is classed as a Vodka (at least in the NH liquor stores) and is preferable as the higher the % alcohol the better it works) but it definitely is NOT "Safe" - there are risks of infection, and so on...
Another thing to consider is that given Dysphoric's medical history from the first post, if they show up in the ER or a doctors office w/ testical damage I'd expect them to be very suspicious and ask a lot of awkward questions about "self harm"... Docs in general are much more aware of the idea of people injecting things in order to get fixed these days than they were in 2014, so reportedly you are far less likely to get away with doing it these days....

I just did a quick StartPage check and got a lengthy list of places, not sure where most are from the link descriptions, but I saw mention of Plano and Austin as starters... (there was even a mention of some in UTAH!)
I also saw a mention of the "Transgender Education Network of Texas"
https://www.transtexas.org/ (Yes, Eunuchs are considered "trans"...) and I would think that they might be a good place to reach out to since they will almost certainly have a list of doctors that would be willing to do the job... So no need for doing risky self harm tactics it seems...

Thank you for trying to be helpful, but you’re failing to take into consideration the limitations of my insurance plans.
What I really need is helpful ideas on how to destroy my testicles in the safest manner.
Everything else that’s been said hasn’t been useful.
