Keelee Macphee- Informed Consent SRS/Orchiectomy

For those looking to get an orchiectomy or SRS, I recommend checking out Keelee Macphee. She’s in Durham NC. She performs both orchiectomy and MTF SRS(vaginoplasty) on an informed consent basis. I believe she’s the only surgeon who will perform a vaginoplasty without letters.

Dr. Arnkoff did my orchiectomy which came out great, but I plan to utilize Dr. Macphee’s services when I’m ready for a scrotectomy( she does these too) or vaginoplasty- I haven’t yet decided which way I’m going to go between those two procedures.

I had the pleasure of talking with her and have found her to be very knowledgeable and attentive. Her website

According to her website, she follows WPATH standards which require letters. The following is copied from her website.....

"Following the Standards of Care set forth by WPATH, Dr. MacPhee"

Her website does and always has had the above statement about WPATH. If, however, you contact them, you’re likely to be told as I was, in writing that Dr. Macphee doesn’t require letters. I think the WPATH website statement is a boilerplate one, possibly as an opening to use discretion on a case by case basis.

I have a sneaking feeling WPATH will sanction her from using their terminology on her website if they find out about it. You can't say you follow their standards, then not follow them.

THANKS!!! That's is good news and makes my day to find another doctor who doesn't play gatekeeper and she is one state over and not across the country or in some god-forsaken place. That is so awesome, just walk in as an intact and walk without..... I will definitely give her a call once this virus shit is over.

I definitely recommend contacting her practice via email or phone. Her assistant, Sue is very helpful and prompt. The medical consultation can be done remotely via Skype as was done with mine. My original plan was to have a vaginoplasty but I opted for the orchiectomy due to the much shorter recovery time. She’ll perform an orchiectomy but she does them under general anesthesia, in a hospital. I preferred having it done in an office under local which is why I went with Dr. Arnkoff for this procedure.

I just found a GP that doesn't get creeped out that I am a eunuch.

I'm confused by her wanting to do orchiectomy in the hospital under general anesthesia. The only reason I could think would be that she does them via the inguinal canal. That approach is how urologists are trained to take out testicles suspected of having cancer. (It's highly debatable as to any actual evidence that there is any benefit to an inguinal orchiectomy being better for anything other than the doctor's wallet when it comes to cancer - there is no evidence it's worse. There is just no evidence. What exists is a 100-year-old thought from a doctor who felt it might be a better approach).

Otherwise, absent cancer, there is no reason the procedure is more complicated than a vasectomy. Oddly, insurance companies don't pay for hospital vasectomies and urologists routinely do them as an office procedure.

Does she do an inguinal orchiectomy or trans scrotal?

My orchi was performed under a general anesthesia via scrotal incision. There is absolutely no way he would do it with a local. But at least it wasn't done in a hospital, but was performed at his clinics surgery center. Now my scrotectomy was performed by a local, but it was a different doctor. The first one again, absolutely would not do the scrotectomy along with the orchi. Doctors these day, what are you going to do?. But at least I got it all done. I had to pay out of pocket, the orchi was around $3500, the scrotectomy, $2500.
