If you have not recovered senstion by now it wii most likely not come back. Your dorsal nerve is probably permanently destroyed.I am not a doctor and I hope i am wrong.

Hey @amith john,
Welcome to the forum.
I am not a doctor either, but it does sound as if you've damaged your dorsal nerves.
The dorsal nerves are not the only nerves in the penis. They run along the top of the shaft to the glans.
If they are harmed it will cause numbness. In fact if a man has premature ejaculation, one treatment is to cut the dorsal nerves in surgery
https://www.youtube.com/watch?v=iQz8wvWPFhA This will allow man to last longer before he ejaculates.
Only a urologist can tell you for sure if there is a way to bring back sensation.

Hey @amith john,
Four years is a long time with no improvement on its own. Not being a doctor, I have no way of knowing.
If you are feeling embarrassed to see a urologist, it's understandable. But, keep in mind that dealing with issues of the penis is about every third appointment for a urologist. Also, every doctor has to go through training. They've seen it all. Doctors have pulled barbie dolls out of guys asses that "accidentally sat on them" while naked.
Nothing you say will shock a urologist.
I'd recommend writing it down on a piece of paper, something like this:
"Four years. Prolonged use of a ring to keep an erection during sex. After that I've felt numb. Any way to treat the numbness?"
You will be the least difficult patient of the doctor's day and they won't blink an eye at the use of a cock ring. They've seen it before.
Totally aside, I had a friend who was nurse in the emergency room. He was doing the pre-check in evaluations. A guy came in with an injury to his arm and the bone was sticking out. The patient was kind of ashamed of the injury and said, "I think I might have broken my arm". My friend didn't say anything but thought, "ya think!". Then he said, "we'll get you into a room for the doctor to decide".
What patients find embarrassing is normally just another day at the office for a doctor.

Okay, I've done a lot of banding and in fact I've been dedicated to trying to actually achieving numbness of the penis. When I've compressed the dorsal nerve the numbing that comes from that is total. Even a candle flame directly on the glans cannot be felt. The dorsal nerves will try to recover but it can be very slow (I've heard they can regenerate as little of 0.1cm per month). The longest it has taken me has been two months. In the case of regeneration the sensation doesn't actually come back slowly but rather stays totally numb then sort of "suddenly" (over course of a couple days) returns.
Based on my experience with numbing due to banding and also other attempts to destroy the dorsal nerve such as injecting alcohol, phenol and glycerol, I've always seen regeneration.
So personally I actually think your problem is something else. Full dorsal nerve damage would also prevent your feeling of hot and cold, plus point damage due to a band should regenerate.
I have experienced the feelings you've described though and I think they are biochemical. If a person is on an anti-depressant, at least for some people the sensitivity of the penis will go way down along with the pleasurability. I've experienced this when I was taking Celexa as a cure for sex addiction. Also, if your testosterone is low (and sometimes other related sex hormone imbalances like progesterone or estrogen) you'll get the same effect -- basically the penis loses a lot of sensitivity as well as the ability to get and sustain a strong erection.
So honestly I would consider that maybe you have a brain chemical or sex hormone deficiency. Have you tried taking a DHT-derived steroid like Proviron? It gives me rock hard spontaneous erections and really strong orgasms. Or are you on anti-depressants? Those might be causing it. Actual depression can also affect this sort of stuff.
Hopefully that gives you some hope. I don't think your symptoms match the damage from a single one-hour banding session.

Hey @amith john,
I hadn't thought of what @SpiltDik pointed out about medications. Prescription antidepressants are famous for causing anorgasmia (the inability to achieve orgasm) by decreasing the feeling in the glans.
As well, there are many over the counter medications that can cause the same effect. One of the most common ones is dextromethorphan (used over the counter as a cough suppressant). In fact DXM (dextromethorphan) is so effective at preventing orgasm, that many doctors recommend it to patients with premature ejaculation.
An inability to get an erection after you've had blood flow studies show no reason for the ed can be caused by either medication or a hormone imbalance.
You've seen the urologist (if you don't have your chart and lab work, get a copy of them). Before going for an implant, I'd see an endocrinologist (a doctor that specializes in issues of hormones and glands). Finding an endocrinologist is difficult. Finding a good one is even harder, but worth the search.
The implant would give you sort of an erection. I say "sort of" because your penis would be pumped up, but you wouldn't really have an erection - just a penis extended by a mechanical device. Also, the implant wouldn't give you sensation back in your glands penis (the head or knob).
Is it medication or hormones? I have no idea, because I'm not a doctor. It's very telling that your numb glands penis happened immediately after the banding. This tells me it's likely to be a result of the banding.
I think the reason most of us are saying it's possible to be something else is that one hour of banding a semi flaccid penis is seemingly a short time and should only result in temporary numbness (if any at all).
As always, your mileage may vary.

I had some experience with a severed nerve in my hand that had to be surgically repaired, so learned a bit about neuroregeneration going through that.
From what I read, the re-growth rate is 1-5mm per day (large nerves faster than smaller). But the nerves don't just re-fuse. Instead the distal end (ie part on far side of injury from the central nervous system) slowly degenerates, and the entire length of the nerve has to regrow back from the point of the injury. The OP did not mention his age, but age does have an effect on how well the nerve recovers as well. Older patients may only recover feelings of hot/cold and sharp/dull.
Safe to say that after 4 years, the nerves are going to have recovered as much as they will ever. But I agree with the other posters that there may be other things going on as well that you should look at.
