Been reading about men who've had scrotal orchiectomies who later complain about pain or lumps from the cord ends. Understand that this problem doesn't occur when the testes are removed inguinally, viz. thru abdominal incisions. Haven't seen any posts comparing the two options. I want to get my worthless balls removed. I was in the infamous Prostate Cancer Prevention Trial some years ago and the finasteride has left me permanently and irreversibly impotent. Can any of you readers give me opinions on the two methods? I'd also want to have the scrotum removed afterwards for a smooth appearance. Is that done by the Urologist or do I find a plastic surgeon?

From what I have read here inguinal procedure does take longer to heal but the chords are cut much higher up.

In the UK most orchies are done this way as they remove more tissue (Good for cancer cases).

I don't think you will get your urologist to remove your sack as you will be asking for the remove healthy tissue.

If you are going to do this by a sex reassignment surgeon then I guess both could be done at the same time.

My nuts were surgically removed in two separate operations inguinally. After three months the hematoma had been absorbed and the cords disappeared within the abdomen, and there is no detectable sign of the cords.

I have read that surgically removing the scrotum is recommended in a separate operation from orchie. It makes sense considering swelling after surgery, and likely would make for a flater result, and give a more appealing result when done by a plastic surgeon.. I guess I will find out next year as I plan to have my scrotum removed.

Mine were removed inguinally. I was driving the next morning. Pain was minimal and recovery very fast. I quickly was back to normal. The biggest, longest discomfort was from the bruising in my groin caused by the surgeon’s efforts to push and pull one of the testicals through the inguinal canal. Since my scrotum has no scaring, it still looks natural, even in its shrunken stage. It almost appears like I still have testicles. I am very happy with her surgical skill.

Mine were removed in two surgeries one from each side of the scrotum and there were some bumps at the ends of the cords for a while but they are gone and I cannot tell they were ever there. Healing was very quick also.

I was castrated by SRS Dr. (not urologist) thru the scrot who also removed most of my sac (about 3/4th of it) in same operation. I went back a year later to have the remaining scrotal tissue removed (even the webbing down the base of my cock) as it just looked kinda funny to have small flap of ballsac remaining. Looked like a bomb went off down there. When castrated, my cords were tied and sutured in tiny knots in the remaining part of my sac but after a few weeks they shrank up and disappeared up into my gut. I agree most Drs. will removed the nutz and sac in two different operations but I didn't have any real complications and not much swelling as there was not much left to swell up. Was castrated on a Monday and flew back on Wed. and could have gone back to work on Friday.

Went the inguinal route. Unlike what others claim, I experienced much pain and swelling. It is abdominal surgery, after all. Scrotum is intact, with no remnants present. The two scars are just above the pubic line.

I had a bilateral inguinal surgery 3mo ago. I had pain for 3 days, swelling for 2wks. by now, my surgery lines are barely visible, a bit of empty scrotum still hanging, no cords, no pain. i love the results.

For about a year after my orchi via the scrotum I could feel the stumps of the cords, but as time passed they apparently retracted into the inguinal canal. There was never any pain or discomfort. Unless there is a need I can't see why anyone would want the inguinal route as they cut through muscle which takes much longer to heal. Further the wound would be subject to far more complications.
