No, it isn't a prank gone wrong, this guy did other similar videos showing his testicles. No one has seen his ripped off balls, no one has seen any blood, no one has seen the ambulance and no one has seen him on the hospital bed nor at the emergency room. But it's a good play though.

I think if you really did this you would be at best unconscious and at worst dead from shock soon after. He has full natural facial colour, that sort of thing would make you as white as a sheet even if you were conscious. The controlled writhing is unconvincing and it is clearly a fantasy play to me. I would not try it out to prove him wrong!

I sincerely doubt it real, for similar reasons Paring gives above.
