Seeking a bit of advice
Preamble: I have never had a serious interest in castration for myself, so please keep this in mind when reading and posting replies. This is about reality, not fantasy or a kink.
Oct, 2015 (first week, unsure exact date) - I had found some lumps on my testicles (both) while doing a self examination. I started researching right away. I was concerned, so I kept an eye on them. They didn't go away, as I had hoped (from my research, I found a number of guys that noted that theirs went away after a while). I still have the lumps today, in the same places, as well as a few new ones that a Urologist found at an appointment 11 days ago. I have a great deal of pain on some days (negating work and the gym/training regimen I am on), and then some days it's only a nagging discomfort. If I were forced to say how often/severity, I would have to say the days are pretty evenly split between debilitating pain/nagging discomfort, meaning I seem to have an equal amount of both. The debilitating pain started in mid November.
My problems/symptoms are:
Testicular Pain (both testicles, sometimes more prominent in one or the other, sometimes prominent in both)
Testicles feel "odd" while examining them (squishy in some areas, firm in others)
Pain at times goes up into my abdomen
Slight swelling at times
Sac skin is darker red at times (but not always...I'm part Native, so my skin is normally somewhat red anyway)
Testicles just simply feel "odd", almost as if they are foreign in my body now
Pain during ejaculation, worse pain after (that lingers)
I have been to see a couple different Urologists regarding this problem. One was, to be blunt, ignorant. I won't go into detail on what she said to me, except to say that she assumed I was having anal sex, which is untrue, and that "Men over the age of 35 do NOT get testicular cancer", which is a bald faced lie. While I am gay, I actually have had no form of anal sex for 14 months now, so LONG predating the problems, and to top it off the last time was with use of condoms. The only sexual contact I have had was last summer, and it was purely oral (with me as the giver) with my partner at the time. I am at this point concerned that I am being misdiagnosed with Epididymitis, while something more serious is going on. I suppose it's also important to note that the three separate Urinalysis Tests I've had done have come back clean from any type of infection, and I've had the full gamut of blood work done to check for STDs (although I knew I was safe anyway) which also came back clean.
I should also probably note that I am very careful while at the gym. As a matter of fact, for the past several months, I have limited myself to simple fat burn/cardio exercises on the Elliptical Machine. I have completely stopped weights. I am, and have never been, a hypochondriac (I go so far as to make it a point of staying AWAY from doctors unless I really need to go...pill pushing is reprehensible in my opinion).
Every Urologist I have seen has declared that this is Epididymitis. But, I do not believe they are taking 1) The time table into account, 2) the lack of sexual contact into account, 3) My honesty into account. I'm not a doctor, no. But I'm intelligent and rational enough to be able to do my own research, and validate or invalidate findings. And I know my body. Something just is not right.
I really do not believe this is Epididymitis from an infection. I am exceedingly careful at the gym, and sexual contact has been nill (as a matter of fact, my lack of desire due to these problems has put an end to my last relationship). It MAY be Epididymitis in that there is obvious swelling in my Epididymus, but there is no infection. I would feel it, and between my immune system (which is extremely strong) and the two rounds of Ciprofloxacin I have been on, any infection would have been kicked out of my system already (I routinely shrug off colds/flus due to my strong immune system).
I have one more appointment at my current Urologist. He ordered an ultrasound, to which after finding the lumps (I had to point them out and hold the testicle - QUITE painful - in order for them to take a scan of it), I was told that "They do not appear to be going into the testicles, and are only on the surface of the testicle." This is of little concern to me, as Seminomas and Non-Seminomas (according to the research I've done) can either be growing in or on the testicle. The fact that mine are not growing into the testicle is, well, irrelevant. The prior Urologist tried to tell me that he believed they were just "Calcium Deposits" on the testicle without running any tests. I'm getting VERY tired and cranky with these people. I do NOT approve of the US Medical community's approach to health via pill-pushing. The antibiotics I was on were very unhealthy to my liver (they have, in themselves, caused death to others), and they haven't solved the problem. If I hear "Rx for Ciprofloxacin" one more time, I'm quite literally going to hit the roof.
I truly believe and feel that I'm not getting proper medical attention. On the last visit, I brought up the subject of a Bilateral Orchiectomy to my Urologist, as a means to end this pain. He only nodded, and stated that is something that they only do if there is a life threatening problem. This is Urologist #3, and I'm already thoroughly disgusted with it.
I believe in my case it's definitely warranted. Do I know the effects that this will have on my life? OH YES. And after initially freaking out about it, and after much deliberation and extensive research, I have come to the conclusion that I would be more than happy to have to deal with those side effects rather than have to deal with this pain (and the possibility that this really is cancer which, I might add, can spread to Lymph Nodes quickly if not treated...and then I would be completely fucked).
I'm really uncertain as how to approach this however. From what I've read, even in cases such as mine, a patient does not have the right to demand a Bilateral Orchiectomy. If anyone has any advice on this subject, or further questions, please leave a reply, and thank you for your time.

I think you'll get the most helpful responses if you can clear up the apparent contradiction.
That said, some other testicular conditions that you might want to research are hemoceles, spermatoceles, and hydroceles. These produce lumps also. My hubby has spermatoceles. They don't go away, but they're not serious. Good luck finding what works for you.

I think you'll get the most helpful responses if you can clear up the apparent contradiction.
That said, some other testicular conditions that you might want to research are hemoceles, spermatoceles, and hydroceles. These produce lumps also. My hubby has spermatoceles. They don't go away, but they're not serious. Good luck finding what works for you.

I a surprised that no one has offered an ultrasound scan.
This is routine practice for anyone who who detects lumps or anything odd down there.
I speak with personal knowledge of this.
I felt a very small lump in one of mine a few years back (I was 50 at the time) and straight away was booked to have an ultrasound done.
In my case it was a scratch from a mild trauma that went away.
Go back to your pactitioner and ask for an uultrasound scan.
The scan is a non invasive procedure that lasts about 10 minutes and generaly speaking you are told straight away of the
results.
This is routine practice for anyone who who detects lumps or anything odd down there.
I speak with personal knowledge of this.

There is no contradiction. You are assuming too much. Yes, I am interested in Penectomy - but that, and the goal behind that, is completely different from castration. Do not assume that because a person is interested in one aspect of a fantasy, that they are interested in ALL aspects of EVERY fantasy.

I did state that an Ultrasound was done, and I stated the results, please re-read. My particular ultrasound lasted an hour (from entrance to the room to exit...I wasn't counting the minutes, but it definitely took longer than 10 minutes).

No problem. Your problem sounds similar to mine, however I have quite a few more symptoms than just simply the lumps. I will find out more at my next appointment in 2 weeks...But I am 99% positive his diagnosis is going to be the same as what I've had in the past. Antibiotics haven't worked. And this has gone on for far too long to be due to a simple scratch from a mild trauma.

You mentioned painful urination. Did they do a urine test?
Sometimes, the epididmitis comes from a prostate infection. If you are in Cisco that would clear that up.
The first thing they do in the UK is a blood and urine test.

I actually did not mention painful urination. And I have had no problems with painful urination. I have had, of course, two bladder scans and three prostate examinations, to which no problems were found.
Please clarify "If you are in Cisco..." I am uncertain what you are referring to. If you meant the Ciprofloxacin, yes. I stated I have finished TWO courses of that anti biotic, with no actual results.

Having run a similar gamut to you in this regard, I have only one piece of advice and that is to keep on changing urologists till you get one who will listen and remove the offenders for you. I was very lucky with the third one I saw, he took one look at my ultrasound scans and told me without any prompting that they had to be removed.
I had them out just over 2 years ago and am very happy with the nett result.
Just keep on till you get a decent urologist and get as many scans as you can, it is all an ammunition gathering process. till you finally succeed.
