I didnt know where to put this as I am new here. This is my deal. Im in my early 40s, have 2 kids, a wife and am straight. Although the wife and I fancy nuetered fully passable trannies.

. I have been on HRT since I was 25. If I dont take it my HRT my test is always between 26-100. I had a 0 sperm count do HRT. Our only ability to concieve was after 1 year of hcg use to produce viable /any sperm, which worked.
In my early 20s, I was in a motorcycle accident which pretty much stopped them from functioning. A few years after that, I had a horrible infection in my testes resulting in epidiymitis. Now almost 20yrs later I have been left with a 0 sperm count unless hcg is used, castrated levels of testosterone unless I take HRT, a pain in the balls thats intermitent, 2 peanut sized gonads, and a ball sack the size of a tarp, and genetic markers that put me at risk for TC and prostate cancer.
I have a great sex life with the wife, but after years of watching castrated tranny porn, both her and I have the desire to have the balls and sac removed. We both appreciate the smooth look and love the idea of it. There is also no desire to have any more children.
Any advice? Anyone? Is there anything I can tell urologists by me? They all say the same thing. "Your fine."

Since you have an active sex life, will you continue HRT so as to be able to achieve an erection? Do you desire to maintain the ability to ejaculate?
Is your desire for castration a deep seated desire, or simply your way of attempting to fulfill a fantasy you have seen in porn movies?
Castration is not a surgery to be taken lightly, for once done, it can not be undone. Have you read any of the forums and literature regarding life as a eunuch?
You should give very cafeful, serious consideration to castration prior to making any decisions. If you believe that you still want to be a eunuch, then a simple search on the internet will provide the names of several excellent doctors you can contact.

Hrt will continue. As for ejaculate, 0sperm is fine. Orgasm is orgasm. Living without balls after 20yrs of thought is a no brainer for me. I already know what life is like with low to non existent t levels. Its quite relaxed. Its the shift between high levels of test and none that suck.
I appreciate the concern and advice. After all this time, i can without a doubt make the move.

Since you are hypogonadal and sterile, you could always argue that your testicles are not functional, but merely decorative accessories. You have no need for them and a cosmetic surgeon may be your best bet for having them removed and your scrotum reduced to the size you desire. You will probably need to show the lab results on testosterone levels and have your wife along with you to give her approval, but this is within the range of what cosmetic surgeons regularly do. You just need to search for the right one in your area.
Urologists are trained to concentrate on improving the function of your testicles, not in removing them except for non-curable reasons. And, they will struggle mightily to find some cure for whatever ails you....

Thank you Jesus. I can only see using the hypogonadal/sterile/cancer marker as the right argument. As for the decorative accessories, they need to go. Its just cluttered down there.

1) Take your wife on vacation to Thailand and get it done. If you can't afford Thailand
2) Take your wife on vacation to sunny Atlanta and visit Dr. Fara Movagharnia:

3) Visit an old urologist like Dr. Paul Allen Church, in Boston. Older urologists are sometimes more inclined to do what patients want done.

4) Try multiple urologists and be demanding. Use the cancer issue. Atrophied testicles are more likely to become cancerous. Keep trying until you get it done!
5) Visit a transgender surgeon like Dr. Reed and explain your situation. Dr. Reed might be able to give you a lead to someone in your area who would be more willing to remove your testicles or you could visit him.
