I was looking for the same thing, then came across a similar thread in German. It seems a clinic in the Czech Rep. and one in Poland offer it for 2300 and 1500 EUR respectively, though it is unclear if they do it on demand or require psychiatrist's letter as, alas, is often the case. When I searched Europe for my orchi, I found nothing suitable and I finally settled with a qualified practitioner in the USA (see other threads), no language barrier, a fair price and undertook the op using local anesthetic only (some I contacted wanted to administer an epidural injection).
