They permanently damage the dorsal nerve, apparently 100% success rate.

I'd definitely go for this -- I've always been plagued with being too fast. I can control it but only with drugs (take Priligy a fast-acting SSRI) and also by numbing the penis (I use lidocaine cream before intercourse).

i've never really thought about this, what is considered as premature ejaculation? is there a time scale from when you start humping of whats considered premature?

i think porn has put a strange view on this when you see guys appearing to go on for hours, but what they don't show is that usually they cum 2 or 3 times within that period, but as the viewers we don't see it as that so think we're not normal for only lasting 10 mins sometimes.... or is that abnormal? hmmm i don't know

Well, I'm no medical expert, but I've always been under the impression that premature ejaculation is defined as ejaculating prior to insertion, or almost immediately after insertion. To any guy that has suffered from this, they have my complete sympathy - it's GOT to be frustrating as H@##.

I suffered from premature ejaculation when I was young. Typically, I'd come within a minute of insertion. I think it was more about being anxious to please (plus being extremely excited by the thrill of having sex) than it was about being physically hypersensitive. And I don't think it was anything unusual for a young guy either. I developed greater staying power with practice and I really don't think having surgery would have been a good idea, even if it had been available back in the '70s.
I find myself wondering how many guys are going to pay surgeons to fillet their poor cocks when all they really need is to get into a supportive relationship and learn to relax and enjoy sex.

I suggest a "performance-based" consideration of this matter, rather than a stastistics or prescriptive-based, set clock-time standard.
Thus "premature" would mean sooner that the two (?) persons doing it prefer it to be, same for the word for when it takes longer than they prefer. What else and who else matters?
Seems that it's nice when the person(s) concerned can control when ejaculation happens so as to satisfy their own desires on each occasion, however quickly or prolonged that may be.

Premature ejaculation from medical point of view is when it is measured in seconds. I used to cum before even insertion, now it is within a couple strokes.
I can even cum (full ejaculation) just by thinking about sex. I often ejaculate when exercising (can't do a set of 60 push-ups without cumming).
I can cum multiple times -- record was 11 times in half hour. But that doesn't work well for intercourse as it sort of goes flaccid for a second.
You're right that porn gives fake impression. Most of the guys also not only use viagra, but also use injectible (old school) erectile drugs. Also, if you watch closely, they are often only semi-erect (you can see the girls trying to hold the penis to make it look erect and can see it bending when they're "bouncing").
I can manage it with a lot of preparation. With Priligy taken an hour before, plus a Viagra, plus numbing the penis with lidocaine, I can last up to an hour. However, you can't set all that up for spontaneous sex -- and often I get "stranded" where I've got a numb, viagraed penis but don't get to have sex.
Most of premature ejaculation is related to seratonin levels. That also affects the refractory period (why I can get it up again within a minute). It is why using an SSRI like Priligy helps a lot. My ejaculations are really intense too -- I can shoot over my shoulder when standing. So it is basically a matter of just being extremely easy to orgasm.
Anyway, this would be an interesting procedure to get done. Maybe in 5 years it will be more widely available.

I haven't had a lot of sex in my life. Too little really. My problem was I took too long to cum. This sounds great, but it is most assuredly not.
It would start off with that "oh my god I'm having sex!" feeling of pure pleasure. After a few minutes of pumping, I'd start to get into that pleasant groove of sensation that is the act of sex. The problem is that feeling would quickly start to fade. I had to use my imagination to remain aroused. The very act of sex wasn't enough to maintain me. More and more minutes would pass, and I'd be fighting a losing battle with arousal. The sensation of sex was fading to almost nothing. No amount of my partner and I changing positions, would help.
This isn't to suggest I found my partners unappealing, far from it. I was very attracted to them. Nor should it suggest I had a limited imagination. It is just after a while, my member would just go numb, and no amount of sensation, or imagination would revive it. Frankly, after about an hour or so of this, it stopped being worth the effort. I was just too wore out.
Now the ladies didn't mind of course. An hour or so was about right for them. It did havoc on my psyche though. I could please them, but I couldn't get any pleasure myself, or at least not the release I wanted.
I can still cum, when I want to. Just by myself. I'm usually pretty quick about it. For me it is either very quickly while the initial pleasure is still fresh, or not at all.
Maybe my dorsal nerve has always been defective. Maybe my circumcision as an infant did some damage I'm not aware of. Either way, voluntarily doing that to yourself sounds like a very bad idea.

Well, it is a "lesser of two evils" decision. Premature ejaculation means you can never have intercourse, and never please a woman/partner that way. And that usually isn't pleasing to the guy either. I'd take having one person pleased (i.e. taking too long to cum) over having none.
Also, it is a matter of do you care more about your own pleasure or giving pleasure. That is a personal choice, but I really don't care about achieving orgasm for myself -- I've been there, done that, about 11 times a day for all my life. I still get pleasure from pleasing the partner, and in fact probably get greater satisfaction from that.
I think the bigger issue is if this procedure causes lack of erection. However, it is noted in the article that it doesn't change sensation for the guy, just the reflex action of the orgasm. In other words, you'll still feel the same thing you'd feel except the orgasm doesn't get triggered as easily.
