In 1995, Stock and Kaplan published their success using ketoconazole to prevent postoperative erections in eight patients.1Ketoconazole inhibits the production of testosterone, and has been used by urologists to treat refractory bone pain and impending neurologic injury in patients with advanced metastatic prostate cancer. Ketoconazole is an imidazole derivative and a common antifungal. In the fungus, ketoconazole inhibits 14-demethylation and blocks the conversion of lanosterol to ergosterol.2In humans, ketoconazole inhibits gonadal and adrenal steroid synthesis to varying degrees at several enzymatic steps. Research in humans reveals that ketoconazole preferentially inhibits 17,20 desmolase, an enzyme necessary for the production of testosterone near the final step of its synthesis.

Ketoconazole also destroys your liver if taken repeatedy so use with caution.

Interesting about the erection prevention. I've taken it a few times because back a few years I made the mistake of going to a tanning bed and I guess the person before had some skin fungus which I got (tinea versacolor) all over my back. You take the pill orally and it kills all the fungus on your skin, so it is a pretty pervasive and fairly powerful chemical. I was warned about liver effect and in fact had to get my liver function checked before the doctor would prescribe it. They did not warn me about sexual disfunction, and I didn't notice if it affected me that way.

Impotence is only seen at very high doses. For fungal infections, you typically take 200 mg/day. For the impotence study they used 1200 mg/day.

In that case, the liver damage is way too big a concern. It definitely stresses your liver at anti-fungal doses. Sounds like it is really only viable for a one-time post-surgery purpose. Not something to use for recreational impotence.

Yeah, probably. But everything has side effects, and maybe this is the right tradeoff for someone. Given the dangers though, this is one of those things that needs to be closely monitored by a physician.

Anything that MAY cause liver damage, MUST be prescribed by a physician AND the liver function MUST be monitored. You DO NOT want to damage your liver, unless of course you don't care if you either die, or need and can afford a possible liver transplant.
I experimented with Siterone just to see if the effects of low testosterone would be tolerable. At first, the andropause was intolerable; I call it "testosterone withdrawal". But the side effects of impotence and loss of sexual ability, sex drive, sex thoughts, sex controlling my life instead of me controlling my life, was overwhelmingly more positive than the negative effects of andropause. (I now take a minimal amount of TRT in the hope that this will keep my T level just high enough to avoid low T problems, but not high enough to re-activate my sexuality, and I found out, this is a delicate balance that changes from day to day, so the lower the T supplement the better).
BUT the potential of liver damage from Siterone meant that I would not, and did not use that drug for very long.
FORTUNATELY, on the EA, I discovered the possible benefits of testicle alcohol injections. Certainly not without risk of complications, but, so desperate was I for castration, and could not obtain castration, that it was FOR ME, a risk I was willing to take; better than liver damage at least.
Castration would be the ultimate perfect drug free solution. Alcohol injections are certainly not to be assumed to be safe, but, at least from my own personal experience, I, for one, am, so far, living proof that nothing adverse occurred. (But do not base your decision to do this just because my experience was not a bad one).
Drugs can be assumed to cause any number of unpleasant to harmful side effects. It is a personal decision one must make, since the medical community will not support or endorse anyone who wishes to elect castration.
