After about five years of chemical castration, I decided to give up the pills and get surgically castrated. Did that in April, in a hospital, with general anethesia (I believe all hospitals use a general; you can't hear if your are out, etc). The doctor I used does not require any "letters", but does have a pre-op chat to see if you are a serious candidate. He also requires a recent EKG (I think the hospital requires it). Got one at a local walk-in clinic. The pre-op stuff in the hospital is a bit annoying, but they do the same as for any surgery. Suddenly I was out and then I was awake and it was over. I had the doctor do an incision in each side of the scrotum and take one testicle out on each side. He said that was better than a central incision. They gave me breakfast and after a few hours of sitting around, I got a ride back to the nearby motel. Laid around with ice on my crotch, but was able to get up and go next door (walked slow with short steps and my legs apart). for some dinner. Had no pain and took no pain pills. Went back to the Doc the nest day for a checkup and all was OK. Drove out of town (hey, I was sitting down), and got another motel later on.

In the weeks that followed, I had a puffyness and bruises that moved around the genital area. I don't like ice on my crotch. Puffyness is not painful. My penis actually later shed some skin. I was careful, but not paranoid, and each day things looked a little better. No bandage was used, but the scrotum was sprayed with some kind of medical glue that covered the incisions, but gradually washed off in a couple of weeks. The only surprise was that the cords were swollen and the tied tips were hard and stayed in the scrotum. It took about two months to start receeding. After three months everything was "normal", of course the scrotum is empty and seems about the same size. Don't care much about that. It can be cut off if I want later. I noticed no particular change in my feelings since I was chemically castrated for a long time. I guess I lost some interest, but it let me concentrate on other things. Definitely a more steady state feeling without ups and downs. I believe it will be different for everyone.

It was not that difficult to get this done in a very legal and professional way. Yes, it cost more tha Dr. K, but not a much as many say or expect. I spend a lot less time now figuring out how to get it done and that is a great benefit.

Based on my experience, no one needs to feel it won't happen.

why does it seem most mens testicles are removed via their scrotum? mine were removed via my abdomen/pelvis?

I'd disagree with your statement that hospitals use general anesthetics for orchiectomies. Mine was done with a spinal block so I was awake and talkative during the procedure and had none of the residual nausea that is common with a general, but I did have to request that. I'm sure it is less hassle for them to just use a general but I wanted to be awake.

My doctor also made two incisions but unlike any others I have seen,the two incisions he made were near the top of my scrotum and were horizontal rather than the usual verticle one(s). This allowed for him to remove more of the cords so there was never any remaining for me to feel and it was easier to suture up since the incisions didn't cross the wrinkled skin but stayed parallel to it. He also included a surgical drain through another hole at the bottom of the scrotum so I had absolutely no swelling or bruising.

I also didn't notice any real changes post surgery as I was chemically castrated for nearly 3 years prior to it. My insurance covered the total cost. The total billed was about $30K and they paid about $5K and everyone was happy.

Most non-cancerous orchiectomies are done using the scrotal approach simply because the surgery is simplier and recovery is quicker and less painful

because no abdominal muscles are cut.

how strange mine were not removed because of cancer but were still removed via abdomen. maybe they removed them differently in my country

Your style of castration is called an inguinal entry. It is said to present less likelihood of adhesions since the cords are terminated pretty much at the point they join the other body parts. Quite a few pre-op trannies have it done this way to eliminate scarring of the scrotum in expectation of using it for the vaginal labia.

Mine were removed via the inguinal canal. My Dr. said that there was less chance of problems that way. Mine also was not cancer. And, I did not have to have letters either. He did tests on me for 7 months to find the sopurce of my pain but couldn't, so he agreed to the procedure if I would go on HRT. So, I said yes. Anything to get rid of that pain.

how much trouble was it to talk the dr into doing the surgery

Is you doctor as easy to work with as kimmel was

What questions does he ask during the interview

would you do it all over again..and are you happy with the job he did??

I too had serious testicular pain (orchialgia), mine was from HRT. I had been on high dose opiate medications for nasty headaches and it apparently stopped my normal T production. When I went on T replacement therapy, I had a horrible time.

They first put me on a T patch, but after a couple of months I showed a serious allergic reaction to those--nasty rings where the patch had been that would take weeks to heal. They next tried me on gel, and I broke out in hives the first time I used it. The last resort was injections. The pharmacist suggested checking for allergic reactions before I just did a full dose, and SURPRISE!!! The doc had put a small amount under the skin on my arm, two days later it was red and swollen and itching.

I went through six months of de-sensitization before getting to full dose on the injections. Within another six months, I was getting serious testicular pain. The pain was so bad it put my headaches to shame and could not be controlled by the heavy opiates I took. I spent several months going through a urologist who finally figured out I was having nerve pain in my testicles.

I was then sent to a pain specialist to do nerve blocks--two failed surgeries later, I was begging the urologist for a permanent solution. The pain doctor could implant a neuro-stimulator, but I wanted to avoid that heavy surgery if I could. I was referred to Stanford for better care.

The doctor at Stanford had me do a little bit of checking on other forms of T therapy and discuss further the pain doctor's option, and then he agreed to do an orchiectomy. I was overjoyed to get rid of that pain!!!

My surgery was a horizontal incision across the scrotum, simple bilateral removal, and was also closed with a surgical glue, very few stitches. I had a bit of swelling with it, but no pain, and in a week I was back at work.

I still take T therapy today to keep my energy levels up, but after having got off the pain meds, I am looking into lowering the dose and maybe adding female hormones in there as my wife has been complaining my sex drive is overly high. I figure that I need a minor amount of T for even a low libido, but the other hormones can help keep my energy levels up. At least I won't need to take huge amounts to try and kill natural T in my system!

Probably the worst part of my whole experience was the first urologist I was sent to. Since I have occasional problems with incontinence, I was wearing 'protection' in the form of an adult diaper (I've needed diapers most of my life). The urologist blew off my pain as 'insignificant' so that he could try to control my incontinence. Jerk.
