BANGKOK : Punlop Tongchai is awake for the entire two hours it takes to be turned into a woman on the operating table of a Bangkok sex change clinic, realising a childhood dream.

The 27-year-old Thai cabaret dancer chats to nurses throughout the ordeal under the surgeon's knife, numbed only by local anaesthetic.

Growing up in Thailand, a country with one of the largest transgender populations in the world and surgeons who have pioneered ever cheaper and quicker sex change techniques, Punlop's ambition was always within reach.

For as little as US$2,000 dollars, he became a woman.

But the government is making it tougher for patients like Punlop to undergo the procedure, forcing them to prove they are psychologically fit to change sex.

From Wednesday, anyone wanting to swap gender in Thailand must live as a woman for at least a year, take a course of female hormones, and obtain the approval of two psychiatrists.

Punlop says it is too much to ask.

"It is not necessary. I really didn't want to go through that process, so I decided to have the operation... before the law is enforced," he told AFP before undergoing surgery earlier this month.

Sex changes are outlawed completely in Thailand before the age of 18, and for men aged 18-20 parental consent must be obtained.

Thai gay rights campaigner Nathee Teerarojanapong said the greater legal protections are necessary to guard against gender swaps that too often backfire on those who make an irreversible choice.

"I got so many calls where they said they are so sorry that they did a sex change," says Nathee. "They make a big mistake and they want to come back and be the same. But they cannot!"

As well as the psychological effects of a sex-change and the recovery from surgery, there are also side effects from the hormones that must be taken to hasten the transformation from male to female that can include hot flushes, tiredness, weight gain and loss of muscle and libido.

Days before the operation, Punlop said that as a child he always found dresses and housework preferable to traditional boyish pursuits and by the age of 19 was regularly taking female hormones in a bid to change body shape.

Punlop went to see Thep Vedusit, a Bangkok surgeon who prides himself on his ability to conduct speedy sex change operations without the use of general anaesthetic, and who believes the new rules are creating unnecessary hurdles.

Thep said he had carried out more than 500 sex change operations and was seeing two or three cases every week -- half of them foreigners who came to take advantage of Thai laws.

"I think a surgeon can make the decision who is a transsexual.... Each (transsexual) has a very long determination to have a surgery and I don't think a psychiatrist gives them any benefits," said Thep.

On the day of the operation, Punlop strolls into the surgeon's glass-fronted clinic, set along a busy Bangkok street in the heart of the city's shopping district, after friends recommended Thep's "fully conscious" procedure.

Dressed in a sundress, tracksuit top and sandals with a face free of make-up, Punlop is escorted up three flights of steps to a small, dingy operating theatre, the walls plastered with newspaper cuttings of transsexual cabaret stars.

Thep takes an hour and three quarters to complete the surgery, allowing his patient to leave the same day with all the necessary check-ups completed within four weeks.

"It doesn't hurt too much; I can handle it. How happy am I? Yes, I'm happy," said Punlop.

"For all of my life I've wanted to change. It's been my dream since I was little."

There are different types of sex change procedures, some are supposed to be better than others. For example, the penile inversion technique is probably the quickest, but might not give the transsexual the depth she wants. I think it's also misnamed, because the penis is actually removed, all except a small portion of the glans which is made into a clitoris. They just invert the skin of the penis, not the penis itself. So you simply remove the testicles (Kimmel could do this in half and hour) cut off the penis, sparing the nerve, invert the skin, sew things together and your basically done. I think this can easily be done in two hours by a qualified surgeon.

1.“Penile inversion technique”vaginoplasty (not good candidate for a case more than 200 lbs. and need more depth)

Surgeon uses the "Penile Inversion Vaginoplasty" technique which turns the penile skin "inside out" and uses it to line a vaginal cavity. The penis and testes are removed. A pure penile inversion limits the size of the vagina that can be created depends on the amount of penile skin available,also the vaginal depth is to depend on penile skin on the resting state not from your erectile state.This technique is called SRS with penile inversion vaginoplasty. You should know the depth hase been gained from your penile skin but not from your whole shaft of penis in resting stage not from your erectile stage.This technique will gain fast recovery and better healing.

2."Scrotal Graft Technique" Vaginoplasty

Some patient's penile tissue limit because the length of penis or the Peno - scrotal junction stays in high position, which will limit the vaginal lining or meaning that vaginal depth is limited.The surgeon will use "Scrotal Graft Technique" combination with penile skin to form the vaginal lining to create the new vaginal depth or new vagina, which is a better technique compared to the penile inversion technique.You will get the depth of your new vagina between 5-7 inches owning to your body structure. This is better choice for the case who have limit penile skin but they have more scrotal sac tissue to use.

Most surgeons currently prefer Scrotal skin graft much more than penile inversion and colon graft.

3."Sigmoid Colon-Vaginoplasty Technique" Vaginoplasty( it is not good case for weight over 200 lbs and larger abdomen.)

You will have two choices to do this surgery about delayed or immediate “ Sigmoid Colon-Vaginoplasty Technique” .Or in case of your penile skin is very short and small amount of your scrotal tissue and refuse to get other skin for grafting to line in your vaginal canal or limited in occasionally rare for some cases with healthy and body weight not over 200 lbs with stopping smoking at least 2 weeks before surgery or non smoking person with flat tummy may need to perform "Sigmoid Colon-Vaginoplasty" do in one stage( Immediate) . In some cases that it will be not appropriate cases to do this procedure ,they have been done by this technique in second step after 6 months(delayed technique) such as the case is not bigger tummy , the weight is over than 200 not more than 225 lbs or maybe need to do this surgery later after 6 months.

In the delayed type,it is better for some cases that have ever done before SRS c Penile inversion or Scrotal skin graft with vaginal shrinkage by the time or for some reason that and need to gain more depth with natural lubricant or they ever had overweight and can not do immediately but they will do later.

All of the cases , I will construct the labia majora , sensated Labia minora, clitoral hood and sensated clitoris in the meantime. The surgeon also constructs labia majora and labia minora by using some part of your scrotal and sensated prepuce to create this part of female appearance.I don’t use the delayed type for this procedure.You will gain natural labia major and minora on this occasion.The surgeon constructs a clitoris by retaining a small section of the glans penis with its blood supply and nerves intact, and position this into an appropriate site above the urethral meatus. Since the nerves of glans in phenotypic male are analogous to the nerves of the clitoris in a female which patients can have feeling of erotic sensation like naturally – born female and maybe having this arousement sensation to reach orgasm in some case but I can not guarantee that in all of the cases,this is the special sense.

Your post was very nicely done, though I'm a bit surprised by your extensive knowledge of the GRS procedures. You've obviously done more than a bit of research on this topic.

I was also wondering the same thing myself considering my GRS took about 4-1/2 hours to perform by an extremely skilled surgeon.

Dr. Preecha Tiewtranon the grand old man of Thai SRS surgery, who taught most of the current crop of Thai SRS surgeons, had (has?) a reputation for doing the surgery in something just over an hour but he has probably done more SRS's than any other surgeon in the world.

One thing puzzles me too. Very often in this board we see complains about how there are no doctors to do penectomy and castration. That in the US and euro they may be sent to a mental hosp. etc. I wonder Do they not know of Thailand!? Very often here all is required is the patients' consent, and they certainly have the expertise to do so. So there is no lack of doctors is it not, only perhaps the lack of funds to fly to Thailand.

Thailand has put some new laws/restrictions into place that require ladyboy's to follow more western procedures before they can get transsexual surgery. Whether the new restrictions affects non-Thai's is unknown.

Ladyboys in Thailand will have to live by similar rules that apply for western countries from now on. The medical consul of Thailand has made new rules for people that want to become a “lesser man”. You need to talk to a shrink and doctors and take hormones for at least a year before you cut it.

They did not say if the rules would apply for men that want to become female but are not Thai and that travel to Thailand for a sex change. The medical industry make loads of money from this every year and some doctors have a waiting list from foreign clients. The foreign men face similar rules in their own country so they speed up things a bit by coming to Thailand for a sex change.

Thai ladyboys have mixed emotions about this since many have saved money for more than a year and are more than ready to do a sex change. They don’t understand why they now have to wait one more year to get what they saved up for. But some Thai ladyboys cut it off just to take a work a cabaret singer or become a prostitute and then after a few years they get depressed and kill themselves because they realize the cut is forever.

On a personal note I think that Thailand have done what the rest of the world has done for a long time and sure maybe some ladyboys have to wait one year more but it will sure create less problems in the end. If you cannot wait one year to cut it off then you should not even consider it in the first place.

That's for sure.

I have problems with this. When I read that one person was open for the whole procedure I lost a great deal of belief that this was even possible. For that to be the case, he would have to have had what is called an area bock and, given this area, there's no one area, you are dealing with several and the problems of anaesthetizing all the areas increases exponentially with each area. Beyond that, you have to be able to verify that the drugs are "holding", that the patient is in no pain. IF there is perceptible pain, you have a patient that cannot be stabilized in terms of movement. Every touch will cause an involuntary spasm. I do understand that Thailand is the leader (sic) in these types of surgeries but I also note that all the patients mentioned were "ladyboys" who may have required far less detail or that some previous procedures had been done.

I will remain very skeptical and, should anyone from North America go there expecting this sort of quick fix, they are in for either a bitter surprise or very poorly done work; Surgery takes time, just closing the various incisions on something like this could take more than an hour.
