Want to have them removed

The long and short is that I am a very submissive gay male. I am a total bottom, have never had sex with a female and topped a male once. When having sex I am always on my back with my legs in the air or on all fours receiving the attention I desire.

First of all - Welcome to the zoo!
Thought number one - removal of your balls (and other parts) is irreversible, so a good way is to take a test drive with chemical castration, to look, wherever you like it or not.
Are you aware of side effects?

I am FULLY aware that once it is done, it is a decision that will be with me for the rest of my life. This is what I really want for me. It is not something that I am doing for someone else.
I am a sissy and want to be on the outside what I am on the inside.
Side effects is an unknown to me, along with any pain involved.

I think what your expressing sounds very much like you should be talking to a Transgender Therapist. This is probably more mainstream then you think and while having an Orchiectomy (Removal of testicles) is not unheard of for TS people it might be better to focus on the larger goal of transition. You will probably find that the big picture includes living "female" which sounds like what your doing now. Starting on chemical castration drugs and being put on a female dose of Estrogen. Then after some period of this and being oked by your doctor you can look into having surgery.
Do some searches on this and talk to your Doctor. He/she can probably find a good therapist who can work with you and help you find the best path for you to follow. Good luck!

I would say it depends if you are ready to fully transaition and what exacatley you want, I would talk to the Physician who would be performing the operation.
