Difficult to do. Dificult getting the suction cup up there, and one has to be sure to pinch off the cup hose for removal. Be wary of brown stuff building up on the cup......

Basically, this involves digital manipulation of the prostate gland via the rectum to cause an ejaculation. "Milking" is repetitious masterbation by this method. Some folks are fetishistic about it, others obsessive, and some just curious. In any case, an ejaculation by that means (or by some thing else rubbing the gland) can be very pleasant.
