Stark is wearing a beanie to hide his scar. The scar makes him look like a penis.
And Eugene (called arse-face) is back with the Saint of Killers.

Its refreshing to see Eugene is keeping good company,
Stark is a bit of a "Dick head"
I may have to go through episode 7 again just in case I missed something ????

It's a joke and an "easter egg" from the graphic novels (comic books)...
In the previous episode, the AllFather made a comment about Starr's head scar.
In the comic, Starr has a fight with Jesse Custer and Jesse smashes Herr Stark's head in an elevator and when he recovers, the scar resembles a the head of a penis.
That's not to be confused with the star-shaped scar around Starr's eye. That was from a childhood fight with bullies.
