I'm curious to know what you told your urologist to convince her to remove it since even female ones are trained not to remove them unless there is the possible presence of cancer. From my understanding, atrophied testicles are no enough reason to have them removed or even the fact that they are "nonfunctional" or that they are causing pain or that you are already on TRT due to low testosterone. I'm not convinced that the increased possibility of getting cancer in the atrophied testicle is enough reason to convince the surgeon to remove it for prophylactic reasons. What am I missing? Is it just because females, not being male, often have a different perspective on this and are more willing to ignore protecol?

What you say to your Urological Surgeon can have an enormouse difference in the outcome. This isn't to infer that they wouldn't do a professional job but if they are made aware that post surgery, you'd like to be able to get and maintain an erection then they can work to see if that's possible. For anyone who uses any method of castration other than surgical, what you'll end up with is a crap shoot. If you've injected yourself with whatever in advance, that can make a difference. Once a substance is admitted to the body, no matter the site of injection, there is always the possiblity that it will find other, unrelated areas and have an effect. In a gross example, it's like giving someone an alcohol enema to get them drunk. The anus isn't a proper receptor for the ingredients to act on the nisle (Sp?) bodies that control the flow of blood to the brain but it works just the same. (It's also highly toxic; There have been a number of cases of homicide that were found to be such because one person gave another the alcohol a toxic enema of alcohol, in one case, hot cocoa mixed with Irish Whiskey....).

In advance of my surgery, I had a talk with my surgeon on precisely that topic and he said he'd make an effort. Admittedly, I am not young so that has to be listed as a causative factor but...I can and do get erection without other stimulation other than my own brain and can produce ejactulate. Of course, it's not fertile but, at my age, I should have no expectations that it would be.

I went for 6 mo 2 visits about pain. Went and had an ulrasound and found epididmitis and hydrocele. Got results over the phone and ok yes there was a visit to get the antibiotics. So the 3rd visit was the charm so to say. I told the doc it was still in pain and litteraly asked to have it removed.
