Undescended testicles (and surgically implanted ones) can have a higher cancer rate. Not sure if this is a good idea. Why keep your testicles and then take drugs to achieve castration? Seems counterproductive.

Did you see the Stop Cancer program last night. Over 525,000 people die in the USA from cancer each year. 1 in 3, a woman. 1 in 6 a man.
We certainly don't need to do anything that increases the risk of cancer.
Better to just have them out, gone, bye bye.
I prefer to try having an empty scrotum before making the decision to have it removed. Don't relish the thought of 2 surgeries, (nor even 1 surgery), but if I liked, or didn't mind, having the scrotum, why pay to have it removed.
At least for those of us who do want to be castrated, testicle cancer (if it did not spread) would be a blessing; insurance would pay for the operation and it would be quite easy to get it done, and done right. I do feel very sad for men who don't want to be castrated, but have to be, because of any medical condition. Bad scene.

Some weeks ago when I had my castration I also had the scrotal sac removed. It was closed, after the removal by the Urological Surgeon, by a Plastic Surgeon and now, only a short time later, aside from some rapidly fading stitches, there's no clear indication that it ever existed. I'm told that there will be a slightly raised line from the Perineum that will connect with the underside of the penis giving the look of a "natural" body.
I would point out that this sort of "extra" surgical work is probably only available in a accredited medical facility. Those who chose other methods of removal may not have access-as in an emergency room-to this sort of surgeon. It's a basic procedure but not one that most Plastic Surgeons routinely do so I'd want to know not only if they will do it but what their projection might be as to the appearance when finished.
Considering-
This is how I would like to get mine done. I'm sure it adds quite a bit of costs though.:-\

Now that I am serious about going to Dr. Kimmel, I am still wondering if I should keep the scrotum. My scrotum is rather tight and small to begin with, so removal might not be all that important. I think that I will ask Dr. Kimmel when he examines me if it should be done or not. The thought of a smooth underside does appeal to me.

The scrotum in most cases does tend to shrink up over time. Suggest you leave, since as you say it is small. Besides why would you want a big scar there? If you remove it, after castration, at least get a plastic surgeon to do it.
