There are a number of doctors who will do an Orchiectomy. One is Dr Kimmel of course. Do a search on him in the Archive to see the results and some of the debate.
I think the larger question is this. The original poster mentioned being transexual. And this makes it far simpler to have this done as either a step in sex reassignment or even as the final result. I think some m2f transexuals simply stop after the orchiectomy and stay on HRT. If your TS then speaking to a clinic that has experts in this makes it a straightforward procedure.
I think there has been some threads on if being a male to eunuch is a 'valid' sexual identity. I'm not sure the question has a point because there clearly are a number of people who already ARE and have been for bazillions of years. After you get past that point there is a sub debate about what makes a "proper" eunuch. Simple a person without testicles or a person with non functioning testes or ??? Again I think the debate is pointless. We already have living breathing Eunuchs of all stripes. Some might be more purist ie (no HRT) and those that just qualify from lack of natal testicles. Both are still Eunuchs in the wikipedia definition.
Now if you accept my points (other then the one on the top of my head) you can also have people who I guess you could say are truly in transition. A male who transitions to be female and has an orchiectomy and goes on HRT full time and lives as a women. These people have "qualified" as female even tho they may still have a shrunken 100% flacid penis. at least legally in most states. You also have men who loose both testicles due to disease or accident and go on HRT and present themselves as male. I think they have just as good a reason to ask to be accepted as male even tho both are "castrates" / "Eunuchs"
Legal / medical / mind bending twists but if you spell it all out I think its pretty straightforward.

There have been a few lucky folks who have had the procedure done through their regular doctor. One that comes to mind is sag111, perhaps you should read his posts and find out more about how he went about getting it done. I think that given your age you should not have too much trouble finding a urologist to do this for you.

I get my female hormone from walmart also. Even though they say a prescription is only $4, my prescription is for 120 tabs of 2mg, so I get ripped off sort of, they charge me $16 for a refill. Apparently they see a prescription as being only 30 tabs.
