I recently looked at the websites (given on another thread) of some of the clinics in Thailand that do full sex-change operations (SRC). All of them seem to require letters of recommendation from psychiatrists or psychologists. But it may be different just for castrations. Some of them also reduce the size of the "adam's apple", although without shortening of the vocal cords as well (as done by better ENT surgeons in western countries, although it takes several months to recover from) there would be little change in the voice unless "breaking" of the voice has only just started (with only thickening of the vocal cords).
