The medical community really sucks big time. They need to change their act and get things into gear.

I fully understand the need for genital surgery. I was castrated by Spector and had SRS by Suporn in 2001.
You ask where you can get it done. Well, you may have to travel. This is a very important step in your life--very big. Life after my genital reassignment surgery (a.k.a. GRS or SRS) is so much different. And I went all the way to Thailand for SRS with Suporn.

You ask what I have gained. I have been a girl all of my life but since I had boy plumbing, I was raised as a boy. I told my family about me early in life, but they originally had a problem with that. They no longer do. Having had surgery to give me female plumbing, I have full legal rights as a female. Michigan even changed my birth certificate to state that I was born female, because the original assessment ("male"), was wrong.
My family considers me to be their daughter and sister. I have an established career as a female semiconductor engineer. Simply put, I am a woman (I always thought so anyway, but it is legal now).
You ask if you need permission for surgery. Yes, you do. Thai surgeons may be more lenient, but at least you should be on estrogen and have lived as a female for six months. In other countries, the requirements are more strict. But if you are transsexual, then you really are a woman with a birth defect. These requirements can be met.
You ask about cons and you ask about peeing. Well, I spray. I have had my new urethra operated on three times now, but still spray. I can live with that.
Some people asked about penectomy (basically SRS with no vagina). Dr. Bellringer in the UK will do that. He states:

I have not had one second of regret from castration or SRS. Having a female gender identity yet having boy plumbing was difficult to bear. The surgery lets me just be me--a decent human being and a caring female. For TS people, the surgery is very therapeutic and was the best thing that ever happened to me.

I just had facial surgery from Dr. O two weeks ago and look hideous right now, but I am progressing in my male-to-female transition. I will post a new photo when the bruises are gone, but suffice it to say that transition has been the most wonderful thing that I have ever done and it is nearing completion.
And for your question, the genital surgery was very important in my success.
