The Dr. in Atlanta seems to be damn expensive as you must pay for a hospital suite and anesthesiologist as well. Try Dr. Hope Sherie in Charlotte, N.C.. She operates in her own SRS clinic and her fees are very reasonable. I do believe she can also refer you to a local counsellor for the required letter. It may be a bit of a schlep from Arkansas but its a beautiful area of the country.

If you're looking for few or no questions asked, you can try Dr. Aguilar in Mexico. I know of many men who have gone to him to get castrated, with good results. He can castrate, penectomize, or even nullify you should you be at a point you are simply done having anything between your legs.
