I may want an orchiectomy in the future and know of Dr. Arnkoff in Michigan. I was wondering of those who have had one could let me know the doctor you used. Also if you know of any other doctors who do them.
Arnkoff might just be a good option in general. I don't know the general costs down in the lower 48 states, but up here in Alaska my only option charges $6000. I'm MtF trans so I have options other than Arnkoff, but for the time being the plan is to go with him because he's the cheapest I've found, even with the flight costs and all that.

Another problem faced by many Urologists concerns payment unless there is insurance that will cover the surgery which implies it was a medical necessity. The Urologist who did mine was the same surgeon whom I've seen for years about other problems. We'd talked about it and, given my age and some other factors, agreed. I paid in advance-I was not asked to do so-and it was done at a local private hospital. If they had qualms about this, neither he nor I heard them. Would he do this for some one who made an appointment and requested this surgery? No. I was a patient of long standing and he was certain of my motives. In discussing it he said, rather ruefully, he had many men who were fathers to young men who said they'd like to have their boys castrated just on general principle.
