I went to India, Nagpur, to the Kamal Institute of Andro-Urology, 2 flights to get there and £1,200 for the procedure and a short stay. The facilities are a little dated but they looked after me so no complaints. It's not a major city and I was told that to get the procedure in somewhere like Mumbai would have been more difficult because the restrictions/rules are more strictly adhered to. A letter of recommendation would grease the wheels but I'd imagine you could be creative with this if you needed to be. If it is a fantasy that's driving you, personally, I'd give this some serious consideration before you commit.

Fantasy aside, if you want it done, get a doc. Any other way is just too unsafe.

US docs are very expensive and maybe don't do a better job than you can get elsewhere for far less.
