For what it is worth, get the scrotum removed. It does sag when warm, it doesn't shrink as much as hoped, and it is just plain ugly. Wish mine were gone.

Thanks for your perspectives everyone. So far it sounds like no one has any 'smooth look' horror stories. Everyone who has had the smooth look is glad they did and everyone who didn't get it seems to want to have it as a second procedure or already has done.
I've been thinking this through and talking it over for a few days and I think the smoother result is what I'd want if there really are no downsides to it. I really don't want anything reconstructive afterwards.
So no one got pain or loss of sensation from taking the smooth look option? Did anyone who had it done have the 'closing eye' technique my surgeon described?

A real life friend of mine lost his testicles in an accident. They were hit by a 3 phase electrical line! To put it mildly they were cooked. They had no choice with him but to do the "eye" procedure you are describing. The result is that it appears as though he never had testicles. The stitches they used were tiny , and in the end after healing his scar appears to simply be the raphe, (that line down the underside of the penis that continues across the scrotum, all the way to the anus).
He is often asked by sexual partners how it was to grwo up without balls, since no one ever guesses that they were removed.
My own scrotum is tiny, but still gets loose, so I will be having the same procedure this spring.
Oh, and to answer your other question, yes, even and empty sack still gets loose when its warm

Just my two cents, but I've been told "Mederma" is good to reduce the appearance of scars. I think that's the name.
Thanks for that. Mederma is indeed the name and it seems to be available in the UK so I'll give it a try.

Thanks again for the advice everyone. I was recently lucky enough to meet up with a good friend who had an orchiectomy about five years ago. I got to talk in detail about the operation and recovery and even look at the result. I'm feeling a lot happier about everything now, and also happy that asking for a reduction to my scrotum to achieve a smoother look is right for me.
I'll keep you all updated with my surgery date and details of my preparation and recovery.

Wish I had mine removed as well,
Mine is rather largish from being stretched from the 1.5 years of having only one testicle. in the cold it is ok as it is retracted but in the warm it is extremely floppy and large, I was told it would shrink, it didn't.
I am going to a university hospital next week and will ask about having it off. I get rashes in the summer.

The smooth look is worth striving for. Plus it would be rather useful for a future urethral reroute also.

Yes, I concur with Devi. If you leave the scrotum and get a reroute (perieneal urethostomy) they'll do a scrotal lift to get the scrotum out of the way of urine flow. Even though I didn't have much of a scrotum to speak of, the urologist did a scrotal lift and now I've got a bunched up numb scrotum the size of a quarter or just a little more. I am not happy and I will let my urologist know I'm not happy when I see her on Feb. 24th. I'm not sure why urologists are so resistant to removing an empty scrotum except they say that "maybe in the future" you'll want fake balls. Really what's caused this over reaction is transsexuals and eunuchs who changed their minds after getting the surgery they wanted and then suing the doctors for the work that they asked for and received. So now those who want to get castrated or have transsexual surgery in the future have to put up with urologist over protection.
