I'd imagine if the patient is only under a local, a doctor is going to know very quick if they are fully anesthetized.
When these surgeries are done with a general anesthetic, they still inject a significant quantity of localized in the area of the surgery. When I had my penectomy, I was under general, and the notes say the doctor injected 60cc of local anesthetic into my crotch. I was castrated with a LOT less than that.
