You don't really want to know. You'll end up screaming and yelling in horror.
But they can remove your prostate with a telescope type device like a laproscope. It's about 3/8ths of an inch thick. Makes you piss like a horse for a few days until the urethra shrinks.
However, you would go in through the penis. That would mean opening the bladder to get to the testicles and that is bad. It's much safer to open the bottom of the sack, yank those bad boys out, clamp-em, cut em free and suture the cords. That's called an orchiectomy (spelling?)...
Althought a dyson like vacuum device sucking the entire sack off the body might be fun to write into a story. It never losses suction until it takes the moghty orbs and seals the man up with nothing left but a thin layer of skin.

No one should ever try this... and if you think about what it would take to make it actually work, you probably won't want to.
The first problem is that the testicle is not filled with a liquid. It's a tissue like any other part of your body. So you can't just suck it through a tube. You'd have to liquify the inside of the testicle first. Now seriously that would be very challenging even with the aid of an anesthetic. Without it you would go into shock immediately..... unless of course your testicles are in a jar already
If that is the case, what is the point?
So how would you liquify your testicles? Would you construct a laproscopic cuisinart? Perhaps a few whacks with a large mallet or wooden meat tenderizer might do the trick? Just remember at some point the lidocane is going to ware off and you testicles are going to let you know just what they think about what you tried to do.
It's always better to imagine these things than it is to actually try them

The liquification is accomplished by the ultrasonic waves.
Many years ago, I read something about an experimental ultrasonic muscle massager; in the story, they mentioned that it had to be used carefully, since the ultrasonic waves were capable of dissolving the muscles.
I'm not absolutely sure, but I believe that different frequencies of ultrasonic waves will affect different types and densities of tissue; thus, the internal portions of the testicles can be liquified without affecting the denser outer portions.

I do not believe that they stick a tube down and through the penis, doesn't make sense, the urethra goes to the bladder and the testicle chords cannot be reached that way and you would need to puncture the urethra at some point to get to the testicles. The way that the urologist uses the aspirator is to open the scrotum through an incision and puncture the testicles with the ultrasonic hand piece. With a flip of the switch he then begins to suck out the tubuli seminiferi. The testicles have a bunch of tubes in them, check out the following and scroll down to page 625.

There is no real positive reason for a urologist to do a subcapsular orchiectomy accept that the patient has a fear of having his testicles cut off, so to pacify him they suck out the insides of the testicles instead, which leave him with bean sized balls. When you suck the insides out of something it goes flat. He might as well have his testicles removed and replaced with falsies.

According to the European Committee of Medical Ultrasound Safety (ECMUS) "Ultrasonic examinations should only be performed by competent personnel who are trained and updated in safety matters. Ultrasound produces heating, pressure changes and mechanical disturbances in tissue. Diagnostic levels of ultrasound can produce temperature rises that are hazardous to sensitive organs.......... Used occasionally and correctly, ultrasound probably won't hurt you.
Used for long periods of time, frequently, and at high intensity, you might just destroy whatever tissue the ultrasound is aimed at: your brain, a fetus, your testicles, etc. I could find no articles about case studues where ultrasound causied sterility or destroyed the testicles ability to produce testosterone, or destroyed the testicles completely . If it is possible, it might be a very painful experience, but hopefully not.
