A couple thoughts:

First of all, for those who are transgendered, the earlier the transition the better. Why go through male puberty then have to have less than satisfactory attempts to surgically revert oneself?

Secondly, I can't think of any teenage straight boy even in alternate cultures who would consider transsexualism just for fashion or because their culture has "preconditioned them to take orders".

The second article posted goes a bit astray in my opinion. Body dysmorphia is indeed a mental disorder, whether it is anorexia, transgender, steroid abuse, etc. That should not put a stigma on it, but means it should be treated professionally, medically, and as early in a person's development as possible.
