There's all kinds of things you can do.

One of my favourites is when my partner takes my scrotum in her mouth while giving a handjob, and sucks just a bit harder than is comfortable, or grips very gently with her teeth. The sensation and sense of danger are incredibly.

Of course, you can always get a bander, so long as you're sure you can use it properly and get the bands off in a timely fashion.

Those surgical stainless steel needles that get used in S&M games work great on the scrotum. I don't know if that kind of thing qualifies as castration play, but it's certainly a great form of CBT. Needles are good for nipples and penises too. But be sure to get the sterile medical-supply needles, unless you happen to find infections really sexy.
