Greetings...
I am a middle-aged american living in costa rica. I am interested in castration solely to reduce or remove my sex drive. I have tried androcur, but it is just too expensive to do long term.
The health care system is very good here and I am confident I can find a urologist that can do the operation for me But because I am totally unfamiliar with the specifics, could someone help me out: I dont know what to do about the scrotum. The three choices seem to be leave it there, have it removed, or insert fake balls. Could anyone please share their experience with this? Are there obvious drawbacks or advantages to any of the options?
I am partial to the fake ball option, just because i am really only interested in removing the sex drive, not so much in body alteration. In short, i think it would freak me out a bit to not have balls (but not too much). But with fake balls I could just easily forget about the whole thing and enjoy my new desire-lessened life as if it were just some unexplained miracle.
So, heres my question: If the penis shrinks considerably, then I would want to anticipate that and get fake balls of the approriate size. Otherwise things could end up misproportioned and be just as distracting as have no balls at all. So, if someone had a penis of eight inches, could you give me specifics on what the length would be after things settled into their new post-op life? Would I be the size I was when I was twelve (which would be like a 60% reduction in size)? Or would it be only like a 20% reduction in size? Since there are so many eunuchs out there, could you perhaps share your specifics in penis shrinkage (if this is not too outrageously personal)?
Also, I understand the scrotum shrinks as well... argh, this dynamic is just so complicated! Can anyone enlighten me as to their experience as to the above three listed options and/or fake ball size recommendations?
Many thanks!

Re: the three options
Just have your balls removed and a year later decide what to do with your sac.
Keep it simple.

Might I suggest that you read my (admittedly rather lengthy) topic in the "Fantasy and Biography" category. Here is a link to it: http://www.eunuch.org/vbulletin/showthread.php?t=8793
I am recently castrated (two months as of this coming Saturday). I troubled over whether to remove the scrotum as well, and decided to take things slowly. The scrotum (well, my scrotum, that is) did shrink, mainly because it holds less contents now. I am not convinced at all that my penis has shrunk. I describe it more as a bit withdrawn into the body, but even that is minimal if at all. I seriously doubt if anyone would notice the difference in penis size (not that there are zillions of people who would be in such a position in the first place!). Maybe penis size will change as time passes, but it just doesn't seem like that is an issue to fret over. As a castrated male, it's only function, for me, is to pee. Perhaps others are into piercings, tattoos, modifiications and/or S&M type activities related to the penis, but not me. It is just a mechanism to empty the bladder.

I think perhaps Smoothie has some of the best advice. Wait and see how large your cock is. Then decide what to do with the sac, and if it's to insert implants, you'll have the information you need to make them proportional if that's a concern.
I've had an empty sac for going on 7 years now, and hardly any one has ever indicated that they've noticed the lack of balls. They're either very polite, or they really haven't noticed.
