With very few choices to choose from in the U.S. and if you don't want to go to psychiatrist to prove you need to be castrated, then yes, Kimmel is the best choice or only choice. Dr. Alter does require a letter from a psychiatrist, doesn't he?
Yes, he does and I think (My opinion) a sanity check is required by many surgeons for any "cosmetic" surgery. I was told that this is more common as law suits become more insane. Can I just say that I think this is a good step to do? I know I was a bit embaressed to be asked to do this but once I did (And passed with flying colors - much to my surprise) I was glad I had. I was able to get a true outsiders point of view (He agreed with my motives and considered them rational) and felt better about going (trying) forward.
One other surgeon (And yes, you need to have a sanity check from her) is Dr Bowers in Colorado. She does like a zillion sex change operations a year so she knows how to do an Orchiectomy pretty well.
Dr Alter has the added benifit of being a Urologist AND and cosmetic surgeon so experience and talent he has in large quantity.
These are the big spotlight type surgeons. Any large university hospital can do this. You probably want to talk to a Triage Nurse about what you are seeking first to find out if they ever do such things.
