There something called "Adrenal Fatigue" https://www.mayoclinic.org/diseases-con ... q-20057906
That's the extent of knowledge of the subject, you could also google it, this was just the first result, if it even applies.

Interesting, will look into it further... I certainly have an issue with feeling tired most of the time, and plenty of body aches (though most have reasons...) but not any of the other things they mention (actually I have to take meds for high blood pressure) and plenty of stress in my life....

I've been in Testosterone since 2008 when I was diagnosed with low T and Osteopenia. The last couple of years my blood work has shown high testosterone even though my dose has been cut to 600mg per week via injection. My results this time was 1191 ng/dL
This has my endocrinologist wondering if I need to be on Testosterone at all. I pointed out that my balls have shrunk and shouldn't be making any. He said it is coming from somewhere as my dose is so small now.
It is possible my balls are still producing, or started producing again after years?
For now I'm going to go down to 400mg and see what the next tests show.
Any ideas?
I am basically in a somewhat similar boat as you. I chemically castrated myself back since 2009. My total testosterone level dropped to 137 ng/mL (normal range 240-871 for my age range). By 2019, I was diagnosed with osteopenia and was prescribed testosterone gel. I had been using the full dosage of 40.5 mg per day. My testosterone was slowly increasing from a mere 241 in 2016 all the way up to 800 in 2023, all with the same 40.5 mg of testosterone gel per day. I was getting so my libido that I was getting uncomfortably back to the horniness of my pre-castration level. As for my osteopenia, my bone density scans showed stable BMD.
Since Dec, 2023, after discussion of my endo, I reduced my testosterone gel to about 1/2 a pump, or about 10-15 mg/day. My latest blood test shows 353, which is on the low but within the normal range. However, at this level, I often get night sweat. But at least, my libido is more manageable. And hopefully, it is sufficient to stop my osteopenia getting worse.
In other words, I too experienced an unexplained increase in total blood testosterone at a constant TRT level.

Did your endocrinologist have a theory why it was increasing? Mine just thinks my body is producing it. He's just not being specific as to it is being made. I just know of testicles and adrenal glands. He attributes it to having lost a lot of weight.

No. My endocrinologist did not try to make up any explanation. He just said that my T was plenty high, like a teenager. But I was still far from exceeding the upper normal limit. He did not think it was bad. It is totally good for my osteopenia. He only suggested to reduce the dosage when I complain about feeling horny like a teenager.
