Pre-op questions for Eunuchs taking testosterone

Good afternoon everyone,

So this is a rather involved question. To start, I have been suffering from hyperactive cremaster muscle syndrome for years (30+) and the lovely urologists have suggested scrotox (yeah it's a thing????) and Microsurgical subinguinal cremaster muscle release surgery. The former is $1400 every 3 to 6 months, the latter over $17k, and I don't have insurance (joys of self employment), anywho, my boyfriend and I found a clinic in Mexico (offices in Guadalajara & Tijuana) that will perform a bilateral orchiectomy & scrotectomy for $2600. Blood work, flights and meals are on me. There is no letter required, it is a transgender facility, and I get in hotel nurse visits for 3 to 5 days depending on circumstances.

I can't do it in the U.S. because I don't fit into a "gender specific dysphoria" because it's chronic pain and urologists freak out when I say "just cut the damn things off, I'm tired of this shit" (actual words to my last urologist ), I have complete intentions of doing TrT. I just don't know the actual success rate because it's "taboo" to want to be castrated. My boyfriend and I would be highly disappointed if I couldn't "pitch" anymore, sooooo what worked best for testosterone replacement without constant ups and downs. I have surgery scheduled for January 15th, 2024. Gonna get my endocrine bloodwork done for surgery and T lvls. Just want to do my best to make sure all the plumbing works after the procedure.

It sounds like you are going to Dr A's clinic in Mexico? (I forget how his name is spelled) if so several of our members have had surgery with him and been very happy with the care they've gotten.

I'm on E rather than T, and wouldn't be functional even w/ T, so not the best expert, but reports I've seen say that the results are VERY unpredictable and "YMMV" (Your Mileage May Vary) Some have no problems (even a few that are not doing hormones) others can't seem to get the motor started no matter what... Most report less desire (OK for you, not so great for your partner) and that it takes more time and effort to get results. You probably should expect that things will work differently, and be prepared to experiment and find what works best for you.

There are a lot of different ways of doing TrT, probably the one that will give you the most even levels is the daily gels, with the note that you must be VERY careful about cross-contaminating partners or others when using them. Next best is probably the shots on a fairly short (i.e. 1 x Week) schedule...

Even if you can't get the surgery locally, do you have a good endocrinologist that can help you afterwards? (if you are having trouble finding one, I'd ask for a referral from the local TG center as your care is going to be the same no matter why you got ✂️🔪)

I have a Trans friend who is introducing me to her Dr in TX. He performed her transition surgery. I will use their endocrinology team.I have researched a new oral daily pill. My biggest fear is loss of function. I have zero fear of the surgery

Sounds like a good find for the endocrinology side of things. Much harder (no pun intended) to be sure about :dong: function, there don't seem to be any certain methods of keeping it that work for most, although the TrT does seem like it helps.

I think you're headed in the right direction. While no surgical option and outcome is guaranteed, I can relate my experience before I became a full nullo, and spent nearly 10 years where you appear to want to go - still having a penis, but losing your testicles.

The short version is, I was able to perform as you wish to, sexually. After my nuts were gone, I began testosterone replacement, and sexually, I functioned as i did when I had testicles, other than I was then sterile.

So long as you use testosterone you should be able to get erections and perform sexually as you do now, assuming you have no other underlying complications such as erectile dysfunction.

I'm heading out soon to Dr Aguilar in Tijuana. Similar issues -- testicular pathologies and pain, lining up to go on TRT. I'll keep y'all updated.

I am about 1 1/2 years post castration and scrotal removal and on Androgel. I can perform better than before although I also take daily dosage ciallis. I absolutely love the look and feel of no balls and being smooth down under. May I also mention that I am 76 years old. I am healthy and exercise regularly which helps to keep the blood circulating. I can certainly understand your apprehension and there are no guarantees but if you are otherwise healthy, having no balls and taking TRT you should be just fine.

I was over four years ago For much of that time I have been on replacement testosterone with no loss of function. My standard protocol is 100mg/week testosterone cypionate by injection. I don't get a rollercoaster effect by injecting weekly. Some even split the dose and inject more frequently. You shouldn't have any problem pitching!
