My advice would be to talk about the issue with the practicioner who's going to perform the castration. He or she will presumably know what a lot o' other guys have experienced. While simultaneously removing your testicles and sac will add to recovery time, it may not add much, and it may not add to the discomfort that you're going to feel in any event. You've answered the question about perhaps requiring your scrotal skin for other purposes.

Hi folks-

As I had Dr Kimmel do my scrotal removal procedure two weeks ago today, here's my two cents on the subject.

Dr Kimmel did my bi-lat orchi in September of 2004 and due to my own stupidity from the remaining testosterone I decided not to have the

I spent 2 hours on his table listening to the cautery and smelling the smell of barbequed flesh feeling very hot blood/fluid run down my thigh while he cut out adhesions to get the damn thing off. Everything basically adhered to everything else. Not what I had in mind, and he wasnt too pleased either. I asked him point blank if there was anything I could have done differently or better and he replied "Do what Philip did. It's always always best to do everthing at once."

Knowing what I know now I'd rather have spent an extra week recovering from a combined surgery than 2 months extra like I did, 2 extra hours on the table, the added expense from the second trip, and now recovery again!

Dr Kimmel did not make any mistakes either time. So don't even start with that crap or I'll hunt you down. I consider Dr Kimmel a very good and caring friend who just happens to be my urologic surgeon. He's not slow, not forgetful, not making mistakes, he's a great man. He is not retiring right away, and for the community I'm thankful for that.

My advice: Do it all at once and get the dang thing over with. You'll hate how the empty scrotum sticks to you legs anyway.

Your post brings up several issues. There's the obvious one of having everything removed at once if one's undergoing castration. Your thoughts and experience are valuable inputs for others considering the procedure.

The other issue it brings up is whether to even think about not having the scrotum removed? Keeping it would seem to be inadvisable because adhesions stemming from slicing the testicles off the end of the cords will result in everything sticking together inside the empty sac. I have no answer. I'm just wondering why this seems to happen in so many cases?

I'll just add that I've retained my sac, not for any principled reason, but because it was practical at the time. While I like the smooth look of no sac beneath the cock, I've just never felt like undergoing another surgical procedure for the removal even though I seem to be a good healer and healed rather quickly from the simple bilateral orchiectomy procedure. My experience is that there have been few if any problems. My smallish remnant of a scrotum doesn't even stick to my legs. More recently, I had 2 scrotal piercings (rings) added. I had no problems with them either, and many of my friends and sexual partners agree they look kewl. I'll add that the practicioner that removed my testicles cut the cords as high as possible for the simple orchiectomy procedure (through incisions in the sac rather than the radical procedure where the testicles are removed through abdominal incisions). He also made rather small incisions - just large enough to express my rather small testes outside the sac, and he also removed the inner sacs surrounding the testicles.

My 2 cents worth and only that. I continue to believe retaining the sac is a personal decision, and one should just balance all of one's objectives in making a decision. It's not a biggie. That's the initial decision to be castrated.

there are obviously severall oppinions on this issue I for one will go with the recomendation of my urologist ,who has many awards for his work and citations from high government offices, his oppinion is to remove the scrotum at the time of castration because to have it done later is nearly the same ammount of trauma as the castration and scrotal removal together.

thats my .02 and i'm throwing in Dr.Kimmel's .02 as well

That was my exact problem, several years latter while consulting to have a peinal inversion the Dr. said I did not have enough tissue to perform the inversion and very little to create labia. My only option at the time was a colon graft

This is obviously an open question in the medical world then, as my urologist said he thinks it's best to wait, due to exactly the kind of infection problem I had: if something does get infected or yucky (particularly with the cord remnants) and the sac's gone, then when fluid starts building up it won't have anywhere to go, causing a lot more pain and general issues. Of course, he didn't say what you do when removing the sac gives you an infection...

I think that my castration will be complete that with the cut of my scrotum!

I would have wished that my bag be cut at the same time as my testicles but one recommended a time to me after castration to remove my bag.

It was also said to me that it should be seen how the empty scrotum evolves, it is necessary to wait to know if it is necessary to remove all or only one part, according to the importance of the scrotum, among certain boys it can decrease in an important way and make ablation useless!

I'll chime in here. I had very small testicles, as they were badly atrophied. I also have very little scrotal tissue. My urologist felt that it was best to not mix the two procedures, and that I should wait to remove the scrotum later if desired.

I also had a very high cut, ( I know cause I watched), she pulled the testicles far out, and clamped the cords, tied off, stitched, and then severed them. The scrtal incisions were very small, and I can't find them after just 6 months.

What scrotum I have , seems to be tightening up even more, and unless I am very warm is nearly smooth. I did look into removing it at the 3 month point, but the aesthetic surgeon in my urologist's practice would NOT list it as necessary, and the cost was 5 k. For that kind of money , I'll deal with it, and just go on a really nice vacation.

Each to his own, but having had some swelling from a slightly leaky blood vessel, I can assure you that having the scrotum there during that part of the recovery was very important.

For whats its worth, the very high cut has left me , with cord stumps that are well retreated into the inguinal canals, and they cause me no problems.

Occasionally over the first few months, when I was out in the cold for a long time, there was a slight ache, but that faded as soon as I came inside.

If I were equipped to do it again, I would still opt to do just castration, followed by scrotum removal if desired.
