If you're willing to travel, I would check out Dr. Aguilar in Mexico. He will ask a few questions, but generally performs these surgeries (castration on up to full genital nullification) with a minimum of fuss. Don't be put off by the name of the clinic. He seems to do good work and could help you achieve your goal with a minimal hassle.
A few weeks ago, I visited Tijuana and had a nice lunch in the neighborhood in the area of Dr. Aguilar's office. I felt very safe. I went back a couple of days later to try another restaurant. It's an upscale area with many doctors, nice offices, and excellent restaurants, including many plastic surgery centers. When there are plastic surgeons in the area, that means there is money in the neighborhood.
I had my implants changed in Guadalajara, Mexico (to get a brand made in Europe that is not available in the USA - mainly because they are not made here).
I would feel safe having a castration there with Dr. Aguilar (or some other urologist in Mexico - Tijuana, Monterrey, Mexico City, or Guadalajara). The medical care is excellent. Often as, as good as here or better.
When you arrive back in the USA (because of liability insurance of US doctors), you will have trouble finding help (aside from an emergency room) if you do have some complications. Therefore, it's best to make sure you have a plan to get back to Mexico in case of problems.
Best to assume that doctors in the US will not help you because they probably won't.
I had Vaser Lipo in Mexico City last Christmas. In late January, I needed one more fluid draining with a syringe (a low-risk five-minute procedure). I could not find any help and had to fly back to Mexico City. Luckily, it's only a couple of hours from Texas with a nonstop flight. It was cheaper to fly back for that help than find a doctor here (for a five-minute procedure). Unfortunately, no one would help me.
Let me know if you have questions about where to stay in Tijuana. I have visited and had a couple of medical procedures in Mexico. I would not think twice if I wanted to get castrated in Mexico. I wish I had it done there from the start so I would not have needed to go there to get new testicle implants. I hated the ones they used in the United States (Coloplast - one deflated - saline filled).
